Full Time RolePay Rate: $25 per hour (Commensurate with experience)9:00am - 5:00pm - Monday Through Friday 




Social Services Assistant

POSITION PURPOSE: To support the activities of the Social Services department while also assisting with the Admissions processes on an as needed basis


Primary Job Duties

Attends PAN meetings to engage actively in discharge planning.

Serve as a liaison with other departments for discharge planning.

Track status and progress of appeals.

Attend and co-lead care plan meetings.

Manage and coordinate goods and services with off-site companies for durable medical equipment, home health visits, and other services required for discharge.

Assist in the oversight of the LTC unit.

Participates and collaborates with the Interdisciplinary team for compliance and completion of third - party service referrals, resident care meetings and family      communication.

Communicates with residents and families following their admission to the community and ongoingly throughout their stay until discharge.

Participates in the completion and verification of documentation including but not limited to Advance Directives (i.e., DNR, Living Will, Powers of Attorney), Social History, Pastoral Care Requests, etc. via charts audits

Collaborates with admissions department for residents requiring additional documentation related to the continuation of their stay such as PASSR upon significant change.

Collaborates and assist in the admission process when necessary.

Participates in resident care meetings with families and resident and documents as such.

Collaborates with nursing department physicians to assist with resources for specific resident care needs such as but not limited to behavioral health management, therapy services and end of life care.

Assists in timely completion of MDS Sections and updates to Resident Care Plans.

Assists in completing the appropriate documentation relating to transitions

Participates in Resident's medical record review and consults with interdisciplinary team regarding Plans of Care as needed.

Acts as a reference to residents, families, and staff by providing guided resources and interventions as needed.

Provides "Active Listening" for Residents/Families directed at problem identification and solution.

Assists in coordinating Resident Council and Family Support Groups as needed.

Participates in Continuous Quality Improvement and contributes to departmental and facility-wide Process Improvement initiatives.

Assistance with facility special events as necessary.

Participates in manager on duty rotation and as needed.

Performs other duties as assigned


Requirements:


  • Bachelor's degree in social work, Healthcare Administration, or related field
  • 1-2 years in Long Term Care providing resident or social service support.
  • 1-year supervisory experience preferred
