JOB SUMMARY:
Under the supervision of the Senior Director of Community Based Services, the Director of Case Management and Discharge Planning oversees the daily operations and strategic planning of the agency's case management and community liaison clinical and administrative team for the Developmental Disability (DD), Mental Health (MH) and Substance Abuse (SA). Key responsibilities include driving patient care outcomes, managing clinical transitions, ensuring regulatory compliance with Medicaid and licensure, and leading interdisciplinary care to optimize financial and clinical outcomes. Will ensure that discharge assistant plans are developed and implemented as needed and mitigates individual complaints and concerns as they arise escalating as needed.
ESSENTIAL FUNCTIONS: (Maximum of Eight)
1. Provide clinical, program, and administrative supervision of the following Adult Behavioral Healthcare Services (ABHS) programs and Youth Behavioral Healthcare Services (YBHS) programs, which include: Mental Health and Substance Use Case Management, Liaison Services, and Discharge Assistant Program (DAP), as well as Intellectual Disabilities and Developmental Disabilities (IDDD) programs. Provide direct supervision to Program Managers and Liaison staff. Serve as a Licensed Mental Health Professional (LMHP or LMHP-e) to provide supervision for Qualified Mental Health Professional (QMHP) staff as regulated by the Board of Counseling.
2. Ensure seamless delivery of services for individuals who are eligible for ABHS and YBHS Mental Health and Substance Abuse Case Management and DAP services by making staff assignments, monitoring service delivery, compliance with Agency policies and procedures as well as Medicaid and licensure regulations and conducting reviews of particular cases. Maintain documentation for referral follow up and disposition and monitor that previously assigned referrals are scheduled in a timely manner.
3. Support the interviewing, hiring, and training of DD Coordinators, MH/SA Case Managers and liaisons. Provide supervision to assigned staff, including training and performance evaluation(s), and encourage opportunities for professional development. Coordinate and facilitate monthly staff meetings/trainings. Provide leadership role for appropriate intervention in crises and emergencies. Supervise liaison services for adults who are involved with emergency services and ensure timely and quality discharge planning for psychiatrically hospitalized adults. Provide support and training to assigned staff in areas of documentation and charting compliance. Approve and enter leave, mileage, and travel expenses in Kronos timekeeping system, and monitor/approve weekly hours worked to reconcile staff time for payroll.
4. Act as Agency and/or ABHS and YBHS representative in assigned meetings and teams, both internally and within the community. Assignments may include Census Reduction Team, Serious Incident Review Committee, Service Application Team, Community Resource Team/GEO meetings, MH/SA committees, or other duties as assigned. Perform rotation on community teams as chairperson, where applicable, to complete agendas, minutes, case presentations, and meeting facilitation
5. Oversee documentation processes in collaboration with Quality Assurance Coordinator(s) to ensure compliance with applicable Agency policies and procedures and regulatory standards including both Medicaid and licensure. Ensure documentation is person-centered and outcome oriented. Provide supervision and support to ABHS/YBHS and IDDD Program Managers, Supervisors of Case Management and DD, Case Managers, Support Coordinators, and liaisons for performance improvement in areas of documentation as needed. Conduct training in areas of charting compliance, complete reviews of records as needed or requested, and maintain supervision documentation for staff for performance evaluation/improvement plan purposes. Participate in the evaluation of existing services and facilitate development of new services, through short- and long-range planning.
6. Oversee and develop Mount Rogers Community Services (MRCS) Discharge Assistance Program in accordance with the Department of Behavioral Health and Developmental Services (DBHDS) guidelines. Supervise a MRCS representative to work closely with DBHDS representatives on developing DAP plans and identifying hospital discharge barriers and monitor the pool of state mental health funds allocated to MRCS.
7. Participate as a member of the Leadership Team in order to participate in all Programs/Services including long-range planning, the need for new, expanded, or modified services and ongoing program evaluations in order to ensure the highest quality of services to individuals served. Conduct Agency training assigned topics. Maintain status as Agency trainer in assigned topics through continued education and/or additional required training.
8. Participate in development of budgets in assigned program areas and monitor revenues/expenses relative to budget while ensuring established reimbursement procedures are followed. Monitor overall budgets for MH/SA Case Management and DD Case Management services. Review and approve expenditures for family support assistance and monitor overall budget to ensure efficient utilization of funds. Maintain data collection of utilization of funds to be used for statistical reporting purposes.
Minimum Requirements
QUALIFICATIONS:

• Demonstrated knowledge of mental health and substance use disorders, diagnostic categories, treatment modalities, intervention techniques, counseling theories and behavioral health services and application
• Comprehensive knowledge of assessment diagnosis, treatment planning, and documentation
• Skills in program planning and evaluation, needs assessment, and resource coordination
• Thorough understanding of human rights and confidentiality laws
• Demonstrated leadership, team building, motivation, problem solving and conflict resolution skills
• Knowledge of the principles of management and skills in clinical supervision
• Interviewing, observing, assessing and negotiating skills
• Good oral and written communication skills
• Exceptional public relations skills
• Rudimentary accounting or budgetary skills
• Good organization and time management skills

EXPERIENCE/EDUCATION REQUIRED:

• Master's degree in the Human Service field required, at a minimum must be license-eligible, and four (4) years of experience of direct clinical knowledge in providing mental health services.

• Licensed Mental Health Professional (LMHP), preferred.

• Direct knowledge of DD services is preferred.

• Valid Driver's license and a safe driving record is required.
