AG Home Health Care is a non-medical home health care provider in Minnesota, providing a diverse menu of services and supports to people with disabilities and the aging community.

We strive to improve the quality of life for those we serve, while prioritizing their safety and well-being. We employ a team dedicated to fulfilling the highest standards of quality care while encouraging independence.

We are currently looking for a qualified professional to provide individualized home support to vulnerable adults who successfully live on their own with varying levels of support. An IHS/Personal Support Worker provides this support by working 1-on-1 with the person.

The Individualized Home Support Specialist (IHSS)/Personal Support Worker works directly with individuals with physical disabilities, mental illnesses, and developmental disabilities providing skill-building and instructional services to acquire, retain and improve the person’s experience living in the community. Individualized home supports can be provided in the individual’s own home, family’s home, or in community spaces used by the general public, and service delivery is provided either in person or remotely.

Responsibilities:

  • Plan and participate in person-centered activities including coordinating and maintain schedule with individuals served
  • Provide cueing, skill maintenance, guidance, instruction, assistance with activities of daily living, and assistance with coordination of community living activities or direct supervision
  • Periodic individual program support plan review and documentation
  • Prioritize client safety & promote client confidentiality, respect, and dignity
  • Assure that client rights are observed at all times
  • Promote professional and appropriate communications with peers, supervisors, and client team members at all times
  • Complete required training at time or hire and annually thereafter.
  • Participate with in-service programs and training to meet compliance requirements
  • Perform other related job duties and responsibilities as assigned

Qualifications:

  • Must be able to pass a DHS background check
  • Must have a valid driver's license and reliable transportation
  • Must be 18 years of age or older
  • Must have the ability to communicate clearly
  • Must have the ability to document observations of client’s needs

Must be able to assess physical and emotional needs of clients and respond appropriately.
