• Conduct in-depth clinical assessments and develop tailored treatment plans that align with each patient’s unique needs.
• Administer evidence-based therapeutic interventions and engage in solution-focused therapy to guide patients through their mental health challenges.
• Document all interactions and progress in accurate, timely clinical records, leveraging electronic health record (EHR) systems to enhance efficiency.
• Collaborate seamlessly with a multidisciplinary treatment team, including physicians, nurses, and other mental health professionals, to create comprehensive care strategies.
• Provide education and emotional support to patients and their families, aiding them in navigating complex emotional experiences associated with serious illness.
• Stay informed of the latest developments in mental health and hospice care, applying this knowledge to improve treatment outcomes.

This role involves working with a diverse range of patients facing terminal illnesses. By addressing both psychological and emotional needs, you'll play a crucial part in helping clients and their families cope with the challenges of serious health conditions. You will also be tasked with guiding patients through grief, anxiety, depression, and adjustment issues common in hospice care.

• Master's degree in Social Work from an accredited institution, ensuring a strong foundational knowledge in social work principles.
• A valid LCSW license in Texas, enabling you to practice and provide therapeutic services.
• Demonstrated knowledge of solution-focused interventions and trauma-informed care approaches.
• Exceptional communication, interpersonal, and problem-solving skills, allowing for effective collaboration and patient engagement.

• Previous experience working in hospice, palliative care, or related settings is highly desirable, providing a critical understanding of the emotional complexities faced by patients and families.
• Familiarity with electronic health record (EHR) systems to document clinical interactions and treatment plans efficiently.
• Experience in interdisciplinary team environments that foster comprehensive patient care.
