    Job Description

    ***This position will travel between the Kokomo/Peru/Lafayette areas***

    We are seeking a Licensed Practical Nurse (LPN) to help support the continuous enrichment and development of individuals with developmental disabilities in our adult residential facilities in New Castle, IN.

    What You Get To Do:

    • Ensure that each individual receives quality, individualized health care management, resulting in their optimal health 
    • Conducting routine site visits, checking vitals, doing assessments, counseling of medication administration, and documenting progress 
    • Act as a health/medical resource for staff, training staff in the medical arena
    • Conduct audits and reviews of medications
    • Ensure that the company is in compliance with state requirements and organizational policies regarding medical care and records 

    Schedule: 

    • Full-time Monday-Friday with occasional travel required; flexible hours
    • Participate in an on-call rotation for triage of medical concerns for 1 week per month
    Qualifications

    What Makes You A Great Fit:

    • Current  Licensed Practical Nurse (LPN) license in the State of Indiana required
    • 1-2 years experience working with individuals with disabilities or other long-term care is preferred
    • Must be skilled, flexible, self-motivated, dependable, and able to work cooperatively with staff, interdisciplinary team members, and outside agencies
    • Strong verbal/written communication skills
    • Ability to work independently and efficiently
    • Organizational and computer skills 
    • Acceptable background check & driving record check
