About the Role

We are seeking a ACT Program Manager who focuses on the client journey into and out of ACT services, with a critical emphasis on the referral pipeline. This role oversees referrals, intakes, and discharges, and manages the complex case team for admissions, referrals, and outreach purposes. A key responsibility is tracking referrals from the moment they are received until the team makes first contact, ensuring no client slips through the cracks. Within their assigned region, this manager also provides direct oversight to local ACT teams on access and transition fidelity. This position supports ACT teams in locations from Raleigh & Durham.

  • Oversees the client journey into and out of ACT services, ensuring smooth coordination of referrals, intakes, admissions, transitions, and discharges
  • Manages and monitors the ACT referral pipeline to ensure timely engagement, outreach, and follow-up from initial referral through first client contact
  • Supervises and coordinates complex case team functions related to admissions, referrals, transitions, and outreach efforts
  • Ensures no referrals or potential clients are lost in process gaps by maintaining oversight of referral tracking and engagement workflows
  • Provides direct operational oversight and support to assigned regional ACT teams related to access, intake processes, and transition fidelity
  • Monitors and supports team productivity, staffing alignment, scheduling, and overall operational efficiency within assigned regions
  • Oversees foundational operational structures to ensure compliance with administrative standards, staffing requirements, and operational expectations

  • Supervise and support teams responsible for admissions, outreach, and complex case coordination.
  • Monitor operations, staffing, and workflows to ensure efficient services and compliance with program standards.

Qualifications

  • Master's degree in Human Services, Social Work, Psychology, Counseling, Nursing, Healthcare Administration, or a related field required
  • Mental health License in North Carolina (LCSW/A, LCMHC/A )
  • Minimum of 1–2 years of leadership, supervisory, or program coordination experience preferred
  • Knowledge of North Carolina behavioral health service definitions, Medicaid requirements, and community mental health systems preferred
  • Working knowledge of ACTT model expectations and familiarity with TMACT standards preferred
  • Ability to oversee operational workflows, referral management, staffing coordination, and productivity expectations
  • Strong organizational, communication, and problem-solving skills with ability to manage multiple priorities effectively
  • Experience working with interdisciplinary teams and coordinating care across providers and community stakeholders
  • Ability to review operational data, monitor compliance expectations, and support process improvement initiatives
  • Proficient in electronic health records (EHR), Microsoft Office, and operational reporting systems
  • Valid North Carolina driver's license with acceptable driving record required
