You will primarily interact with clients via telehealth platforms and phone communications, synthesizing assessments and care plans that respect individual desires and circumstances. Your responsibilities will include:

• Conducting thorough psychosocial assessments to understand the unique needs and challenges faced by patients and their families.
• Developing, implementing, and adjusting individualized care plans tailored specifically to patients' physical and emotional requirements.
• Providing counseling and emotional support to both patients and their caregivers, focusing on coping strategies for dealing with end-of-life issues.
• Advocating for patients' rights and ensuring access to community resources, ensuring families receive the necessary services and support.
• Collaborating closely with other healthcare providers, including nurses, physicians, and specialists, to create a seamless care experience for patients.
• Maintaining regular check-ins with clients through virtual visits, assessing their health conditions and addressing any new concerns promptly.
• Educating families about grief and coping mechanisms, along with available community services and support networks.
• Documenting all client interactions and updates meticulously in our electronic health record (EHR) system to ensure compliance and continuity of care.

Your primary focus will be on geriatric clients and their families, many of whom are experiencing significant transitions and emotional challenges as they navigate the complexities of hospice care. This population often faces unique psychological challenges, and your expertise will help them find peace, dignity, and understanding during this crucial phase of life.

• Master's degree in Social Work from an accredited institution.
• Active and current LICSW license in the state of Washington.
• Minimum of two years of experience in social work, preferably in a healthcare or geriatric setting.

• Proven experience in hospice, palliative care, or direct work with terminally ill patients.
• Strong familiarity with the challenges faced by families during end-of-life care, including grief and loss issues.
• Solid understanding of community resources available to seniors and their families.
• Experience with electronic health record (EHR) systems to document patient interactions effectively.
