Key Responsibilities

Your responsibilities will include:

  • Conducting thorough psychosocial assessments to understand the unique needs and challenges faced by patients and their families.

  • Providing counseling and emotional support to both patients and their caregivers, focusing on coping strategies for dealing with end-of-life issues.

Qualifications
  • Master's degree in Social Work from an accredited institution.
  • Active and current LICSW license in the state of Washington.
  • Minimum of two years of experience in social work, preferably in a healthcare or geriatric setting.
Preferred Experience
  • Proven experience in hospice, palliative care, or direct work with terminally ill patients.
  • Strong familiarity with the challenges faced by families during end-of-life care, including grief and loss issues.
  • Solid understanding of community resources available to seniors and their families.
  • Experience with electronic health record (EHR) systems to document patient interactions effectively.
