This role is designed to be primarily remote, allowing flexibility for our Social Workers to deliver care where it is most often most needed. While you will engage with clients virtually, you will also be part of a supportive, collaborative team dedicated to exceptional mental health service delivery. Regular team meetings will ensure you remain connected with your colleagues and supported in your efforts to advocate for clients. Furthermore, there may be the occasional requirement for evening or weekend hours based on client needs.

• Conduct comprehensive psychosocial assessments to identify the emotional, mental, and social needs of clients, especially seniors.
• Develop individualized care plans in collaboration with clients and their families, focusing on specific goals and preferences.
• Provide direct counseling and emotional support to clients and their families, assisting them through life transitions and challenges.
• Advocate for clients' rights and facilitate access to community resources, ensuring clients receive the services they require.
• Work in partnership with interdisciplinary teams, including nurses and other healthcare professionals, to enhance the comprehensive care experience for clients.
• Monitor clients' welfare through regular visits, assessing their living conditions, and addressing any safety concerns promptly.
• Lead conflict resolution efforts among family members regarding care strategies, ensuring cohesive support structures for clients.
• Educate clients and families on community resources, support groups, and programs to foster greater social engagement and improve quality of life.
• Maintain detailed records, documenting assessments, care plans, progress notes, and other relevant interactions.
• Stay abreast of current social work practices, regulatory requirements, and ethical standards to ensure compliance and best practices.

As an LCSW at Fort Behavioral Person-centered Therapy, you will focus primarily on supporting older adults and their families. The unique needs of this demographic require a strong understanding of aging-related issues and a compassionate approach to mental health care. You will empower clients to manage anxiety, depression, and the emotional complexities associated with aging, providing them with the tools to regain control of their lives.

• A Master of Social Work (MSW) degree from an accredited institution.
• Current and valid state licensure as a Licensed Clinical Social Worker (LCSW).
• At least two years of experience in social work, preferably in geriatric or community service settings.
• Strong understanding of social work principles, aging-related challenges, and available local community resources.
• Exemplary communication skills, both verbal and written, to effectively interact with clients, families, and caregivers.
• A proven ability to work independently while also valuing teamwork and collaborative decision-making.

• Experience in developing and implementing care plans tailored to individual client needs.
• Background in emotional and psychological support for elderly populations.
• Familiarity with remote health service delivery methods and tools.

We offer a competitive salary range of USD 70,000 – 100,000 per year, reflecting your experience and expertise in the field.
