About Direct Care Home Health Services

Position Summary

The Director of Nursing is responsible for the overall clinical operations, quality, compliance, and performance of the home health program. This leader ensures regulatory readiness, outstanding patient outcomes, timely documentation, and efficient care coordination while fostering a highengagement culture for field and officebased clinicians. The DCS provides strategic and daytoday leadership to RN/LPN field staff, Clinical Managers, and HHAs, and collaborates closely with Intake, Staffing, QA, Billing, and HR to achieve operational and financial goals.

Leadership Scope & Reporting

Reports to: Executive Director/Administrator. Direct reports may include Clinical Managers/RN Supervisors, LPNs, and HHAs (per agency structure). Partners with: Intake/PCC, Scheduler/Staffing, QA/Compliance, Billing/Revenue Cycle, HR/Training, and Business Development.

Key ResponsibilitiesClinical Governance & Quality

  • Own the clinical model of care and standards of practice; ensure evidencebased, patientcentered care.
  • Lead the Quality Assessment & Performance Improvement (QAPI) program; analyze trends, implement PDSA cycles, and track results.
  • Monitor OASIS accuracy and timeliness; drive documentation quality and clinician accountability.

Regulatory Compliance & Survey Readiness

  • Ensure ongoing compliance with CMS Home Health Conditions of Participation (CoPs), DC DOH requirements, and payer policies.
  • Maintain survey readiness at all times; lead planofcorrection (PoC) activities and validation audits.
  • Protect PHI and uphold HIPAA; enforce infection prevention standards and field safety protocols.

Staffing, Capacity & Access

  • Oversee clinician staffing capacity and coverage (RN/LPN/HHA); address gaps and hardtostaff cases.
  • Partner with Scheduling/Staffing to meet SOC/ROC/Recert timeliness and continuity of care goals.
  • Support recruitment, onboarding, precepting, and retention; optimize caseloads and travel efficiency.

Care Management & HighRisk Oversight

  • Review highrisk cases (e.g., complex wounds, infusion, high readmission risk) and ensure escalation pathways.
  • Champion transitions of care and resumptionofcare processes to prevent avoidable ED visits/readmissions.
  • Conduct case conferences and field ridealongs; coach clinicians and close care gaps.

Education, Competency & Supervision

  • Establish orientation and ongoing competencies for RN/LPN/HHA; coordinate inservices and skills validation.
  • Provide clinical supervision and performance feedback; manage corrective action where needed.
  • Promote a culture of safety, accountability, and recognition aligned with agency values.

Documentation, EMR & Revenue Integrity

  • Ensure timely, complete, and accurate documentation (OASIS, orders, visit notes) to support billing and compliance.
  • Resolve QA issues and EVV exceptions; collaborate with Billing/Revenue Cycle to prevent denials and expedite recoupments.
  • Standardize EMR workflows and templates; drive adoption and train on updates.

Performance Improvement & Analytics

  • Track KPIs (e.g., SOC timeliness, visit completion, OASIS timeliness, QA correction rate, HHVBP measures where applicable).
  • Use data to guide coaching and process improvements; present trends and action plans to leadership.
  • Support growth initiatives by aligning clinical capacity with referral patterns and payer needs.

External Relationships & Community Presence

  • Foster relationships with hospitals, SNFs, providers, and payers; support resolution of escalated clinical issues.
  • Represent the agency in professional forums; uphold the brand promise: Compassionate Care Delivered.

Pay Range

$120,000 –$140,000 annual base (commensurate with experience), plus incentives tied to quality, timeliness, and operational goals.

Qualifications

Required

  • Active RN license in the District of Columbia, in good standing.
  • Bachelor of Science in Nursing (BSN); Master’s degree preferred (MSN, MHA, or related).
  • 3–5+ years of progressive leadership in home health or a closely related setting.
  • Expert knowledge of CMS Home Health CoPs, OASISE, HIPAA, infection prevention, and DC DOH requirements.
  • Demonstrated success driving quality outcomes, documentation timeliness, and survey readiness.
  • EMR proficiency and strong Microsoft 365 skills (Outlook, Teams, Word, Excel).
  • Valid driver’s license and reliable transportation for field supervision and site visits.

Preferred

  • Home health leadership experience in the District of Columbia.
  • OASIS certification (e.g., COSC) and/or coding credential (e.g., HCSD).
  • Advanced wound/ostomy certification or infusion program oversight experience.
  • Experience with EVV and home health EMRs (e.g., Homecare Homebase, WellSky/Kinnser, AlayaCare).
  • Bilingual or multilingual skills.
