Job Description
The Director of Nursing (DON) LTC/PA-C is a professional who:
  • Is responsible for the overall functioning of long term care and post-acute care nursing departments.
  • Demonstrates the clinical and leadership knowledge and experience to direct and support Perham Living Skilled Nursing Facility designed in the Household Model of Care.
  • Ensures the nursing department is staffed with an optimum workforce made up of highly skilled staff with demonstrated competencies in various job classifications that work as a team and consider Perham Living as an employer of choice.
  • Provides the highest quality of evidenced-based care and promotes wellness for all residents/patients while creating an experience of home that uplifts resident choice and opportunities for a meaningful life, and promotes resident, patient, and family satisfaction.
  • Practices optimal fiscal management; in accordance with current applicable federal, state, and local standards, guidelines and regulations.
  • Practices continuous quality improvement initiatives.
  • Ensures an optimum post-acute care experience by realizing all earned revenue while preparing rehabilitation patients to resume their life in the least restrictive living arrangement as timely as possible.

Qualifications
The DON must have a current RN license to practice in the state of Minnesota. A MSN is preferred. This position requires a minimum of 3 years of geriatric experience in long-term care/post-acute care and proven experience as a nurse leader. The DON must demonstrate leadership abilities; have the necessary management skills to direct optimal utilization of people and resources, the design of work processes and new services; possess strong critical thinking and problem solving skills; be able to prioritize responsibilities; be flexible and able to work under stressful situations; possess good verbal and written communication skills (English); be self-directed and able to work autonomously.
Must be able to pass Background Study through the Minnesota Department of Human Services.
Additional Information
Full time position includes: health, dental, paid time off, retirement plan, life and long term disability insurances.
Work Schedule: Generally, Monday Friday; 8:00 a.m. to 5:00 p.m., with some evenings and weekends.
Pay Range: Competitive pay offered.
