Your Role & Responsibilities

  • You will provide comprehensive care to seniors following clinical standards.
  • You will collaborate daily with your care team to ensure coordinated, high-quality care.
  • You will anage referrals, hospital/SNF coordination, DME, and home health services.
  • You wil participate in daily huddles.
  • Document care with support from quality-based coders.
  • Participate in on-call rotation and support growth initiatves at your center.
  • Ensure compliance with all licensing and accreditation requirements.

What We're Looking For

Required:

  • Master's Degree as a nurse practitioner or completion of a physician assistant program with board certification.
  • Active, unrestricted NP or PA license in the state of practice.
  • Two years of experience in value-based care, geriatric or primary care, managing adults aged 65+ with chronic conditions
  • Computer proficiency (EMR, email, etc.).
  • This role is considered patient-facing and is part of Humana/Senior Bridge's Tuberculosis (TB) screening program. If selected for this role, you will be required to be screened for TB.

Preferred:

  • Experience in Medicare Advantage settings.
  • Active DEA license and Medicare/Medicaid provider numbers or ability to obtain DEA license and/or Medicaid and Medicare numbers
  • Familiarity with HEDIS quality indicators and Medicare guidelines.

The compensation range below reflects a good faith estimate of starting base pay for full time (40 hours per week) employment at the time of posting. The pay range may be higher or lower based on geographic location and individual pay will vary based on demonstrated job related skills, knowledge, experience, education, certifications, etc.


 

$123,800 - $170,300 per year


 

This job is eligible for a bonus incentive plan. This incentive opportunity is based upon company and/or individual performance.
