We are currently in search of an experienced Psychiatric Rehabilitation Program (PRP) Direct Service Worker. Direct Service Workers provide assessment and treatment services that promote recovery, full community integration, and improved quality of life for persons who have been diagnosed with a serious and chronic mental health condition that seriously impairs their ability to lead meaningful lives. Direct Service Workers are required to provide collaborative, person directed and individualized services that focuses on helping individuals develop skills and access resources needed to reduce or eliminate the maladaptive behaviors that stem from a mental health diagnosis.

Principle Responsibilities and Tasks

To perform this job successfully, the PRP Direct Service Worker must be able to satisfactorily perform each essential function listed below:

  • Work scheduled shift of Monday - Friday 6am - 2pm or 7am - 3pm
  • Serves as a liaison between the PRP and Mental Health ("MH") Departments (at all CCG Sites) assuring that there is consistent collaboration and a constant flow of referrals
  • Coordinates and processes referrals received from MH staff assuring that the PRP Screening, Letter of Acceptance and the Initial Authorization Request Form is completed
  • Coordinates the referral process with the Revenue Cycle Staff and assures consistent communication between the two departments
  • Performs monthly chart reviews and completes documentation of the process and provides feedback to staff and leadership
  • Audits record keeping in SAMMS to assure that holds are being managed and that services and forms are completed and signed
  • Conducts the initial assessment to gain an understanding of the client's level of functional impairments, strengths, abilities, needs and preferences
  • Develops an individualized and patient centered treatment plan that describes the goals, objectives and interventions of the treatment process
  • Implements the delivery of services through onsite (clinic setting) and offsite (home/community) services that includes:
    • Teaching and Instruction
    • Encouraging positive change
    • Skill demonstration and practice
    • Promoting individual wellness, self-management and recovery
    • Assisting in the restoration and/or improvement of self-care and social skills
    • Facilitating effective strategies for strengthened independent living and the achievement of self-sufficiency goals
    • Exposing individuals to academic, vocational and employment training and resources
    • Facilitating culturally relevant activities directed toward improved social relationships
  • Provides services through face to face interactions and telehealth
  • Performs documentation requirements related to treatment planning, monthly summaries, progress notes and other assigned documents
  • Facilitates monthly collaborations with the client's assigned mental health therapist and treatment team
  • Maintains the clinical records and compiles documentation required by CCG and governing agencies including the signing of all forms and services
  • Participates in staff meetings, program clinical team meetings, clinical supervision sessions and in-service as required
  • Understands and supports the CCG philosophy and treatment modalities
  • Ensures confidentiality regarding client issues, background and treatment
  • Exhibits clear, professional, respectful and effective communication when interfacing with families and external contacts
  • Skilled in working independently and implementing time management strategies
  • Exhibits a high degree of ethical conduct, integrity and confidentiality
  • Ensures client information per HIPAA standards
  • Maintains case files within electronic health record (EHR)
  • Performs other duties as assigned

Qualifications and Requirements

  • At least a high school diploma or equivalent. Associates Degree or higher preferred
  • Previous experience working with emotionally and behaviorally challenged adults or experience working in a recovery-oriented program
  • Must be experienced and proficient in using an electronic health record system
  • Strong interpersonal skills
  • Ability to travel in the local area and perform field work
  • Must have validated driver's license and a dependable vehicle to travel between sites and provide offsite services
  • This position must be actively working toward certification provided by the Commission on Rehabilitation Counseling

The pay for this position is $19 - $22 per hour. Factors, such as but not limited to, geographical location, relevant experience, licensure, education, and skill level may impact the pay for this position.
