What You'll Do:
  • Direct Care: Assist members with daily activities (ADLs/IADLs) like personal hygiene, dressing, and meal prep.
  • Monitoring: Keep an eye on member health and report any changes to the Nurse or Care Manager.
  • Support: Provide medication reminders and ensure prescriptions are refilled on time.
  • Documentation: Complete simple daily logs for each visit.
