Job Summary:

We are a busy and growing family practice clinic specializing in primary care, seeking a dedicated and compassionate Primary Care Provider to join our dynamic team. Our practice offers comprehensive healthcare services, including preventive care, chronic disease management, and acute care.

As a Primary Care Provider, you will play a vital role in delivering high-quality, patient-centered care. You will work collaboratively with our multidisciplinary team to support patient wellness, manage chronic conditions, and address urgent healthcare needs. We are looking for a full-time provider who is passionate about primary care and committed to professional growth within our expanding practice.

Key Responsibilities
  • Perform comprehensive evaluations of patients, including physical exams, acute care, preventive care, and chronic disease management.
  • Diagnose, treat, and manage a wide range of primary care conditions, including hypertension, diabetes, respiratory illnesses, and more.
  • Provide care coordination for patients transitioning between healthcare settings, including hospital discharge follow-ups and post-acute care.
  • Educate patients and families on maintaining proper health, preventive care, and managing chronic conditions.
  • Provide urgent care services, including treatment for minor injuries, infections, and other acute medical issues.
  • Manage and monitor value-based patients, ensuring compliance with care plans to achieve quality benchmarks.
  • Utilize HEDIS measures to evaluate and improve patient care, including closing care gaps and reporting on quality metrics.
Collaboration and Teamwork
  • Work as part of an interdisciplinary healthcare team to coordinate and optimize patient care.
  • Collaborate with nurse practitioners, physician assistants, medical assistants, and administrative staff to enhance patient outcomes.
  • Participate in case discussions and team meetings to develop comprehensive care plans.
  • Engage in initiatives aimed at improving HEDIS scores and ensuring high-quality care for value-based patients.
Patient Education and Communication
  • Educate patients on lifestyle modifications, medication management, and preventive care practices.
  • Address patient concerns and provide guidance on maintaining a healthy lifestyle.
  • Document patient visits accurately in the Electronic Medical Records (EMR) system and ensure timely updates.
  • Effectively communicate the importance of preventive care and chronic disease management, especially for value-based patients.
Quality Improvement
  • Contribute to practice initiatives aimed at improving patient care quality and satisfaction.
  • Participate in quality assurance activities to maintain high standards of care.
  • Stay current with evidence-based practices and implement best practices in primary care.
  • Monitor performance metrics related to HEDIS and value-based care, implementing improvement strategies as needed.
QualificationsEducation
  • Medical degree (MD or DO) from an accredited institution.
Licensure/Certification
  • Valid medical license to practice as a physician in the state of Florida.
  • Board certification in Family Medicine or Internal Medicine. Physicians with ACN licenses are welcome to apply.
  • DEA license and current CPR certification.
Experience
  • Minimum of 2 years of experience in a primary care setting preferred.
  • Experience managing value-based care patients and utilizing HEDIS measures for quality improvement.
  • Familiarity with interdisciplinary team-based care and chronic disease management.
  • Experience with diverse patient populations, including pediatric, adult, and geriatric care.
Skills and Abilities
  • Strong clinical and diagnostic skills with a focus on evidence-based care.
  • Excellent communication and interpersonal skills to build rapport with patients and team members.
  • Proficiency with EMR systems (experience with Athena is a plus).
  • Ability to work effectively in a fast-paced, patient-centered environment.
  • Bilingual (English/Spanish) is highly favorable.
  • Demonstrated ability to provide compassionate, patient-focused care.
