Case Manager II - Waiver Case Management (St. Louis County)

People Incorporated is seeking Case Manager II to join our Waiver Case Management - (St. Louis County) team to work with clients who are diagnosed with chronic mental illness or other ongoing health conditions. We connect people with services by developing person-centered Support Plans and authorizing services that help them live as independently as possible and integrated within their communities. Waiver Case Management involves more behind-the-scenes service coordination than traditional case management Our Case Managers see clients twice annually and develop long term relationships by developing dynamic service plans. Our staff operate autonomously, creating their own hybrid schedules after completing initial training. We value work-life balance and have manageable caseload sizes that serve 30 individuals on average!

Hours: Full-Time, Monday- Friday, business hours 8am-4:30pm

Location: St. Louis County, MN - Hybrid

Hiring Range: $24.25 - $25.75 hour

Hiring Bonus: $1,000 to external candidates

•    Attending bi-annual meetings with clients to monitor services
•    Develop and maintain a collaborative relationship with clients serving as a role model for clients by demonstrating positive, proactive and respectful communications
•    Assessing client needs and ensuring continuity of care. 
•    Maintain regular contact with collaborative partners.
•    Making referrals and authorizing services based on client needs and personal outcomes.
•    Maintaining records and documenting all services provided.
•    Excels at documenting all interventions, interactions, and relevant information to the Support Plan consistent with professional and organizational standards
•    Must reside within the metro area

To perform this job successfully an individual must be able to perform each essential duty satisfactorily. The requirements listed below are representative of the knowledge, skill, and/or ability required.   • Passing all background and requirements for facility and position; and
  • The ability to pass the screening required by Federal Human Services Office of Inspector General to assure that person has not been excluded from working in the Health Care Programs which receive federal funding.
  • Must have a valid and current driver’s license and auto insurance.


Requirements for All Case Manager II positions:

  • Valid driver’s license, excellent driving record and current auto insurance
  • Education and Experience Required for Targeted Case Manager II position:
  • Bachelor’s Degree in behavioral science or related field
  • 2,000 hours of supervised experience working with adults with severe and persistent mental illness
  • OR
  • Master’s Degree in behavioral science or related field.


Preferred Qualifications for Targeted Case Manager II position:

  • Experience providing direct services to adults with serious and persistent mental illness
  • Experience with case management and commitment proceedings
  • Knowledge of Recovery, Motivational Interviewing and Person-Centered approach

Education and Experience Required for Hennepin and Dakota County & Saint Louis County Waiver Case Manager II positions:

  • Bachelor’s Degree from an accredited 4-year college with a major in social work, psychology, sociology or a closely related field;
  • OR
  • Bachelor’s Degree from an accredited four-year college with a major in any field, AND
  • One year of experience as a social worker/case manager/care coordinator in a public or private social service agency


Education and Experience Required for Ramsey County Waiver Case Manager II positions:

  • Bachelor’s Degree in Social Work, Special Education, Psychology, Nursing, Human Services or other fields related to the education or treatment of persons with developmental disabilities or related conditions, AND
  • One year of experience in the education or treatment of persons with developmental disabilities or related conditions
