Position Details
Shift: Day 5x8-Hour (08:00 AM - 04:30 PM)

Floating Expectation: Float to the Laguna Facility
Job Responsibilities
  • Provide care coordination for acute medical care.
  • Participate in the development of treatment plans.
  • Organize and participate in family meetings.
  • Conduct needs assessments and arrange Durable Medical Equipment (DME), skilled nursing facilities (SNF), and ambulance transport independently.
  • Assist patients with discharge planning and coordinate care for patients experiencing various issues, such as mental health crises, homelessness, and substance abuse.
Requirements
  • Experience: Minimum 2 years of acute medical care management experience, specifically in care coordination.
  • Licensing: Valid California state license required; pending licenses accepted if active upon the start date.
  • Travel Experience: Required.
  • Charting System: Proficient in Epic.
  • Knowledge of: CMS regulations, HIPAA guidelines, OSHA standards, and The Joint Commission/core measures.
  • Certifications: Relevant certifications in care management.
Preferred Qualifications
  • Experience in Trauma Level I or II facilities.
  • Background in disability case management, appeals and denials, and worker's compensation case management.

  • Daily Patient Census: Up to 25 patients

Additional Information
Staffing operations occur 7 days a week with a variable 5-day workweek. Days off may vary and may not be consecutive, although every effort will be made to ensure consecutive days off when possible. All candidates are expected to commit to work on 3 of the 5 holidays: Thanksgiving, Christmas Eve, Christmas Day, New Year's Eve, and New Year's Day.
