This position is fundamentally remote-based within a school setting, facilitating flexible engagement with clients in their homes or community locations in Waco, TX. Our work hours, primarily Monday through Friday from 8:00 AM to 4:30 PM, allow for a structured schedule, although occasional evenings and weekends may be required based on client needs. The role demands adaptability to ensure that we meet the diverse and dynamic requirements of our clients effectively.

As a Social Worker at our community center, you will undertake the following key responsibilities:

• Conduct comprehensive psychosocial assessments of seniors, analyzing emotional, social, and mental health needs to address their overall well-being.
• Collaborate closely with clients and their families to create customized care plans that align with individual goals, preferences, and circumstances.
• Provide ongoing counseling and emotional support to help clients navigate transitions such as health changes, loss of loved ones, or relocation.
• Act as an advocate for clients, ensuring they have access to necessary community resources and services, thereby enhancing their support network.
• Coordinate with interdisciplinary teams, including nurses and caregivers, to streamline service delivery and improve care outcomes.
• Perform regular client visits to monitor well-being, assess living conditions, and promptly address any safety or health concerns.
• Facilitate family dynamics by improving communication and management of conflicts that may arise regarding the care of senior clients.
• Educate clients and their families regarding available resources, support groups, and programs that promote community engagement.
• Maintain accurate documentation of services provided, assessments conducted, and client interactions in compliance with relevant standards.
• Stay updated on current trends in social work practices and regulations to ensure ethical and effective service delivery.

The social worker will focus primarily on seniors in our community, addressing their unique emotional and psychosocial challenges. This client demographic often faces isolation, health fluctuations, and the stress of navigating caregiving situations, making the role pivotal in offering support and practical solutions tailored to their unique circumstances.

• Master's degree in Social Work from an accredited institution.
• Current and valid licensure as a Licensed Clinical Social Worker (LCSW) in the state of Texas.
• A minimum of two years’ experience in a social work role, particularly in community or geriatric settings, is essential.
• Strong understanding of aging-related issues and available community resources.
• Excellent communication and interpersonal skills, with an emphasis on empathetic engagement with clients and families.
• Proficiency with electronic health record (EHR) systems, along with basic computer literacy.

Experience in a community-based organization or with client assessments in a geriatric context is preferred. Familiarity with support programs and knowledge of local age-related resources can enhance your effectiveness in this role.
