Job description

Support Circle of Life's Bereavement Services program by providing counseling services to those suffering from grief associated with the loss (or anticipated loss) of a loved one. May see clients of all ages and backgrounds.
Requires LMSW or LCSW certification through state of Arkansas without restrictions. Minimum of one year related clinical experience in a hospice, medical or mental health setting. Demonstrated clinical experience in the areas of life-threatening and chronic illness, grief and loss, individual, family and group counseling. Demonstrated knowledge of grief modalities, therapeutic interventions, and best-practices. Ability to maintain confidentiality, be discerning with information and use good judgment in handling sensitive client or other information. Strong presentations skills and knowledge of community referral sources.
SPECIFIC DUTIES AND POSITION REQUIREMENTS
  1. Provide one-to-one and/or group counseling to those grieving the death of a loved one (past or future) or living with a life-threatening or terminal illness.
  2. Prepare and maintain electronic clinical case files utilizing designated client tracking software; record client data and submit weekly clinical session logs/reports.
  3. Participate in staff, team, and relevant community meetings; contribute knowledge and experience to nurture a learning environment.
  4. Provide on-site and follow-up (both in person and via telephone) clinical interventions/consultations for hospice patients and families, including children and teens.
  5. Provide counseling to persons in crisis and critical incident, stress management and bereavement services.
  6. Maintain communication with primary care physicians, clinicians, and/or other individuals significant in each care plan.
  7. Conduct initial assessments, maintains and updates treatment plans as needed.
  8. Document contacts made with outside agencies and professionals as well as making referrals to community resources.
  9. Provide bereavement support for community members, debriefing and participate in community presentations.
  10. Consult clinical supervision on an as needed basis; Provide supervision for MSW interns.
  11. Utilize the Electronic Medical Records system on a daily basis.
  12. Participate in program development of the department.
  13. Provide back up to the social work team.
  14. Support Circle of Life staff and community professionals with grief and loss issues as needed.
  15. Follow bereavement policies and procedures.
  16. Accountable for direct delivery of bereavement care services provided by Circle of Life Hospice.
  17. Assess, plan, implement and document appropriate interventions.
  18. Demonstrate compassion, empathy, and patience when interacting with patients, families, co-workers, and members of the public.
  19. Represent Circle of Life to the community in a positive manner.
  20. Demonstrate flexibility, versatility and a positive attitude in integrating additional duties.
  21. Perform other duties as assigned.
