Mount Zion Health & Rehab Center is looking for an MDS Coordinator to join our growing team!

The MDS Coordinator is a key member of the Mount Zion Leadership Team who will integrate and coordinate a resident-centered nursing strategy with a strong focus on quality care. The MDS Coordinator’s primary responsibility is overseeing the resident assessment process, including individual assessments, MDS completion, and care planning. This role helps coordinate resources and supports the overall continuum of care for residents.

MDS Coordinator Responsibilities:
  • Provide accurate documentation of services to ensure proper reimbursement from all payor sources

  • Ensure needed resources are available and quality care is delivered to all residents

  • Coordinate the development, review, and revision of resident assessments and care plans

  • Collaborate with interdisciplinary team members to support resident outcomes

  • Maintain compliance with all applicable regulations and documentation standards

Qualifications:
  • Current Illinois RN or LPN license required

  • Long-term care experience preferred

  • 1–2 years of MDS experience preferred

  • Knowledge of long-term care nursing practices and procedures

  • Strong communication, organizational, and documentation skills
