Company Description

We are seeking a compassionate and experienced Level 3 Licensed Practical Nurse (LPN) with DDI experience to provide high-quality care in a residential or community-based setting. The ideal candidate will have hands-on experience with G-tube feedings and tracheostomy care and a strong commitment to supporting individuals with developmental disabilities.

Job Description

Responsibilities:

  • Provide skilled nursing care, including G-tube feedings and tracheostomy care

  • Monitor and document client health status and report changes as needed

  • Administer medications and treatments per physician orders

  • Collaborate with the healthcare and direct support teams to ensure safe, person-centered care

  • Ensure compliance with DDI and state regulatory requirements

Knowledge & Abilities

  • Strong clinical assessment skills and the ability to develop and implement care plans.
  • Excellent communication and interpersonal skills.
  • Proficiency in managing and administering medications.
  • Ability to work independently and as part of an interdisciplinary team.
  • Proficiency in electronic medical records (EMR) systems and Microsoft Office Suite.
Qualifications

Requirements:

  • Active LPN license in MD

  • Proven experience with G-tube and trach care

  • Previous experience working with individuals with developmental disabilities (DDI)

  • Strong documentation and communication skills

  • Reliable, professional, and compassionate

Additional Information
  • Shift

    Monday - Friday 9am - 5:00pm and flexibility to accommodate individual needs and schedule variations.
