Bilingual (Spanish-Speaking) HCBS Coordinator
TheraCare PLLC – Arizona
Full-Time | In-Office Position | Phoenix, Arizona

TheraCare PLLC is seeking a highly organized, bilingual (English/Spanish) HCBS Coordinator to support our rapidly growing Home and Community-Based Services (HCBS) division. This position plays a critical role in coordinating services for Arizona DDD members receiving Habilitation, Attendant Care, and Respite services.
Our team is dedicated to helping individuals with developmental disabilities achieve greater independence while supporting families throughout Arizona.
Position Summary:
The HCBS Coordinator will be responsible for coordinating services, recruiting and onboarding providers, communicating with DDD Support Coordinators and families, managing authorizations, ensuring documentation compliance, and maintaining provider schedules and caseloads.
The ideal candidate is bilingual, detail-oriented, compassionate, highly organized, and experienced working with DDD, ALTCS, HCBS, or social service programs.
Responsibilities:
Service Coordination:
  • Coordinate Habilitation, Attendant Care, and Respite services for DDD members.
  • Communicate regularly with families regarding service needs and provider availability.
  • Match members with qualified providers based on location, schedule, language, and service needs.
  • Monitor service authorizations and assist with obtaining new or updated authorizations from DDD Support Coordinators.
  • Track service utilization and help prevent lapses in services.
Provider Recruitment & Onboarding:
  • Recruit new Habilitation, Attendant Care, and Respite providers.
  • Conduct provider interviews and assist with hiring.
  • Guide providers through onboarding requirements.
  • Verify required documentation including:
    • Fingerprint Clearance Card
    • CPR/First Aid Certification
    • Article 9 Training
    • Direct Care Worker requirements
    • Driver's License and Auto Insurance
  • Assist family-referred providers through the onboarding process.
DDD & Compliance Responsibilities:
  • Maintain accurate records in accordance with Arizona DDD and HCBS requirements.
  • Review provider documentation for completeness and compliance.
  • Monitor Electronic Visit Verification (EVV) compliance.
  • Assist with incident reporting and follow-up.
  • Ensure provider files remain current and compliant with DDD and AHCCCS requirements.
  • Stay informed of DDD policy changes, vendor blasts, and HCBS requirements.
Relationship Management:
  • Build strong relationships with DDD Support Coordinators.
  • Respond to family and provider concerns promptly and professionally.
  • Serve as a liaison between families, providers, and DDD.
  • Conduct routine check-ins with providers to ensure service quality and retention.
Administrative Duties:
  • Maintain provider schedules and availability.
  • Track open cases and staffing needs.
  • Assist with service reports, audits, and compliance reviews.
  • Support growth initiatives and community outreach efforts.
Qualifications:
Required:
  • Fluent in both English and Spanish.
  • High School Diploma or GED.
  • Excellent communication and customer service skills.
  • Strong organizational and multitasking abilities.
  • Proficient with Microsoft Office, Google Workspace, and web-based systems.
  • Valid Arizona Driver's License.
Required:
  • Experience with Arizona DDD services.
  • Experience with Habilitation, Attendant Care, Respite, or HCBS programs.
  • Experience working with individuals with developmental disabilities.
  • Knowledge of DDD authorizations, EVV systems, and provider compliance requirements.
