Job Title: Residential Care Provider (RCP)

Job Code: SSS010/ Client Support Worker or T6001/Support Services Worker

Shift: Full-Time (40 Per Week / PRN - 19 hours per week)

Job Hours: Times May Vary

Base Location: Onsite - Conyers, GA.

Division/Department/Program: Southside AGR (IDD)

The Residential Care Provider monitors general resident activity to observe and report behavior/progress of individuals with severe developmental disabilities living in a group home or other community residential setting and provides individualized skills training according to residents' needs.

Under direct supervision, assists with daily living activities in a residential facility. Provides training of basic personal and social skills, simple academics, or work skills to consumers/students in a state group home, workshop, or supported employment environment.

Duties & Responsibilities:

  • Assist individuals with Developmental Disabilities in basic daily living skills.
  • Carry out a person-centered Individual Service Plan (ISP).
  • Ensure individuals are dressed neatly and appropriately and are always groomed according to VPH policy.
  • Assists and encourages individuals to care for immediate personal needs such as toileting, washing their hands, and eating.
  • Teaches individuals daily skills such as household chores, laundry, cleaning, and washing dishes, as appropriate according to their individual plan.
  • Transports individuals to work, day programs, doctor appointments, shopping, recreational activities.
  • Assist with scheduling medical appointments.
  • Assists in the implementation of individual and behavioral plans and provides feedback for plan goals.
  • Assists individuals in the pursuit of recreational, social and community activities based on their preferences and individualized treatment plans.
  • Assists and encourages individuals in the establishment of social networks.
  • Promotes participation and involvement in community outings.
  • Ensure everyone's rights are protected.
  • Treats individuals with dignity and respect
  • Maintains the residence in a clean and organized condition.
  • Other related duties as assigned.

Minimum Qualifications:

High school diploma/GED and six (6) months of relevant job-related training or experience.

Preferred Qualifications:

  • Experience working with IDD population.
  • Certified Nursing Assistant certification (CNA), Direct Support Professional (DSP), Direct Care Professional (DCP) or Patient Care Technician (PCT) preferred.

Requirements/Competencies:

  • Must pass a pre-employment drug screen and subject to random drug screens.
  • Must have a valid Driver's License and a clean Motor Vehicle Record in accordance with company policy.
  • Must pass a criminal background check (including fingerprinting).
  • Requires TB Evaluation at pre-employment and annually.
  • Must be able to lift 20 pounds.
  • Requires long periods of sitting/standing.
  • Must have the following technical skills - Word, Excel and Electronic Health Record (Care Logic).
  • Must be willing to transport residents in the company van.
  • Must demonstrate the ability to maintain professional boundaries with clients in a residential treatment setting.
  • Must be able to lift and carry groceries and supplies and move furniture for cleaning.
  • Must be able to assist disabled individuals on/offvehicle and secure seatbelts.
  • Must be able to use a gait belt and mechanical lift to lift and position individuals, as needed.

Note: Some positions may require a valid driver's license.
