Job description


Be a part of a world-class academic healthcare system, University of Chicago Medical Center, as a Sleep Nurse Navigator, in our Neuroscience department. This position is a 100% onsite opportunity.
The Sleep Nurse Navigator serves as the primary nursing and care coordination resource for Sleep Surgery patients, providing in-clinic support to the Sleep Surgery physician(Mondays all day, Tuesdays all day and Wednesday in the Morning).Responsibilities include preparing patients for clinic visits and procedures by reviewing medical records, ensuring all required testing and appointments are completed, and coordinating care throughout the patient journey. The Navigator communicates with patients via phone and MyChart to schedule appointments, obtain necessary clinical information, coordinate outpatient procedures, and facilitate sleep studies and follow-up visits. Additional duties include managing prior authorizations, reviewing adult and pediatric sleep study requisitions, determining appropriate sleep study types and advanced testing requirements, pending orders for provider signature, and identifying high-risk patients for provider review. The role also serves as the primary point of contact for Pediatric ENT coordination, Restless Legs Syndrome (RLS) iron infusion therapy planning in collaboration with pharmacy and infusion teams, and clinical trial patients requiring inpatient sleep studies or home sleep testing. The Sleep Nurse Navigator may also assist with CPAP pressure adjustments for select remote patient monitoring patients and works closely with Behavioral Sleep Medicine and Psychology programs to ensure coordinated, multidisciplinary care. The Sleep Nurse Navigator is expected to maintain competency in core Sleep RN functions, including DME and CPAP supply coordination, sleep study order entry, communicating sleep study results, triaging patient inquiries related to sleep therapy, and processing CPAP-related orders in accordance with approved nursing protocols when operational support is needed.
Essential Job Functions
  • Provides care coordination & patient education to patients in MSK service line.
  • Supports discharge planning for the MSK service line.
  • Supports transitioning of trauma/OON patients to providers within network
  • Facilitates resolution to patient care gaps and monitors follow up/completion by both proactively reaching out to patients and addressing direct phone calls and messages from patients.
  • Acts as liaison for patients, families and their multidisciplinary clinical team.
  • Educates/reinforces education with patients and families on disease, treatments, side effects, adverse reactions, and reportable signs/symptoms based on multidisciplinary care plan.
  • Clinical documentation review to improve LOS.
  • Develops and/or coordinates development of patient education materials, clinical pathways, and quality studies.
  • Serves as primary point of contact for patients discharged from the hospital to address questions and patient care needs to prevent readmission or an emergency room visit.
  • May be responsible for direct patient care
  • Performs other duties assigned.

Required Qualifications
  • Registered Nurse with a current Illinois license.
  • Certified Nurse in Specialty area preferred or in process.
  • Minimum of two years clinical nursing experience with demonstrated clinical competence in patient care, teaching and management of patients.
  • Must have general computer skills and be adaptable to changing working situations and work assignments.
  • Excellent interpersonal skills required, with specific ability to communicate effectively with patients, family members, and physicians.

Preferred Qualifications
  • Certified Neuroscience Registered Nurse (CNRN) through ABNN or RST certification through ABSM
