Director of Nursing (DON) – Assisted Living Program (ALP)

Pay: $35.00–$40.00 per hour, based on experience

Orchard Brooke Assisted Living, part of Culture Care Senior Living, is seeking an experienced and compassionate Registered Nurse (RN) to serve as our Director of Nursing.

The Director of Nursing plays a major role in carrying out that mission every day.

We are looking for a strong clinical leader who will oversee all nursing, clinical care, medication management, resident assessments, UAS assessments, care planning, and personal care services throughout Orchard Brooke.

This is an opportunity for an RN who wants to lead a team, build strong relationships with residents and families, and help ensure the Culture Care standard of care is seen and felt throughout the entire community.

What You'll Do

  • Provide leadership and oversight of all nursing, clinical, medication, and personal care services

  • Complete and oversee UAS assessments, resident assessments, and individualized care plans

  • Ensure care plans accurately reflect each resident's needs and are updated as conditions change

  • Oversee medication management, medication administration, MAR documentation, and medication-related systems

  • Supervise, support, educate, and develop nursing and personal care staff

  • Monitor resident health status and ensure timely follow-up on changes in condition

  • Coordinate care with physicians, pharmacies, hospitals, home care agencies, therapists, and other healthcare providers

  • Maintain strong communication with residents and families regarding clinical needs and changes in care

  • Ensure all clinical documentation is complete, accurate, timely, and compliant

  • Participate in resident admissions and help determine appropriate levels of care

  • Oversee clinical components of new admissions, hospital returns, and changes in resident condition

  • Ensure compliance with applicable New York State Department of Health regulations, company policies, and standards of care

  • Conduct clinical audits, quality assurance reviews, and ongoing clinical monitoring

  • Assist with infection prevention, staff education, competencies, and clinical in-services

  • Participate as a key member of the Orchard Brooke leadership team

  • Hold the clinical team accountable to the Culture Care standard of compassion, professionalism, responsiveness, and resident-centered care

  • Help create a community where residents, families, and team members genuinely feel that Care is Our Culture

What We're Looking For

  • Current New York State Registered Nurse (RN) license

  • Nursing leadership or management experience preferred

  • Assisted living, senior living, long-term care, skilled nursing, home care, or geriatric experience strongly preferred

  • Experience with UAS assessments and care planning strongly preferred

  • Strong knowledge of medication management and clinical documentation

  • Excellent leadership and staff development skills

  • Strong organizational skills and follow-through

  • Ability to communicate effectively with residents, families, physicians, and team members

  • Someone who is comfortable being visible and involved throughout the community

  • A leader who believes high standards and compassion should always go together

  • Someone who takes ownership and wants to continuously raise the standard of care
