Job description

Job Title:Occupational Therapist

Department:Therapies

Location: 1555 Long Pond Road, Rochester, NY 14626

Hours Per Week: 40 hours per week

Schedule:Day shift, flexible hours between roughly 7:00am-4:30pm

SUMMARY

As an Occupational Therapist, your strong attention to detail and excellent evaluation skills are crucial to successfully restoring or improving each patient's optimal level of function, independence and quality of life.

ATTRIBUTES

  • Three (3) to six (6) months of previous experience preferred

  • Compassionate, kind and patient focused

  • Excellent communication and customer service skills

  • For ElderONE Employment: 1 year experience with frail elderly population preferred

RESPONSIBILITIES

  • Evaluation & Development. Perform a complete initial evaluation of referred patients using evaluation protocols, standardized tests and clinical observations; develop and implement treatment plans, including short- and long-term goals and methods to achieve them; adjust treatment plan and goals as needed

  • Documentation & Communication. Maintain complete documentation of patient evaluation, treatment, progress and discharge status; counsel and teach patients and families; coordinate care plans with other involved health professionals and discuss changes in treatment programs with MDs

  • Employee Development. Assist with the supervision and guidance of new personnel, assistants, students and volunteers as assigned; develop and conduct in-service training programs

EDUCATION:

BS: Occupational Therapy (Required)

LICENSES / CERTIFICATIONS:

BLS - Basic Life Support - American Heart Association (AHA), OT - Occupational Therapist - New York State Education Department (NYSED)

PHYSICAL REQUIREMENTS:

M - Medium Work - Exerting 20 to 50 pounds of force occasionally, and/or 10 to 25 pounds of force frequently, and/or greater than negligible up to 10 pounds of force constantly to move objects; Requires frequent walking, standing or squatting.
