Job description

This is a day shift 6am-2pm, rotating days schedule with rotating days off.
Nurses are core members of the Clinical Support Team (CST). Their role on the CST includes assessing and monitoring the clinical status of the elders and delivering evidence-based clinical care, both directly and through the Care Partners and other CST members. The CST nurses report directly to the Director of Nursing but work in collaboration with the Guide and Care Partners.
Duties and Responsibilities:
  • Obtain an update on each elder from the House team members during each shift
  • Visit each elder during each shift to monitor clinical status, and to determine if the clinical care needs of the elder are being met
  • Clinical assessments should be elder-specific, consider the primary diagnosis and risks specific to the individual elder
  • Assess each elder for unresolved/untreated pain. Plan and implement pain management interventions when appropriate, especially related to: skin integrity, bowel and bladder status, including continence and bowel movement monitoring, mentation, ambulation status, nutrition (weights) and hydration, and the effects of medication
  • Identify any clinical teaching needs of the House team members, elder or family. Provide the teaching and complete the documentation
  • Monitor standard precautions (i.e., presence of gloves, soap, towels, )
  • Administer treatments and medications
  • Monitor overall adequacy of the care plan
  • Review compliance with the care plan
  • Maintain emergency supplies and equipment
  • Conduct physical assessment of each elder
  • Support personal care and model excellent service
  • Mentor, coach and educate the SMWT to increase clinical knowledge and elicit collaborative planning.
  • Support MDS assessments, infection control, and care planning activities as directed

Qualifications:
  • License: Active LPN licensure in the state of Texas. Active CPR certification (BLS) required.
  • Experience: Minimum of 2 years nursing experience, preferably in a skilled care or long-term care setting. Experience in a person-centered care environment is preferred, with an understanding of the Green House (GH) model.
  • Education: Graduate of an accredited nursing program (LPN).
  • Skills and Abilities: Strong understanding of the Green House (GH) model and person-directed care principles; ability to collaborate and make shared decisions with interdisciplinary team members, focusing on resident-centered care; friendly, compassionate, and empathetic approach to care, promoting a positive, supportive environment for residents; excellent communication skills, both written and verbal, to effectively document and share patient information; strong critical thinking and problem-solving skills, with the ability to assess, plan, implement, and evaluate nursing care; ability to adapt to a self-managed, team-based work environment, demonstrating flexibility and support for colleagues; ability to assess and respond to medical emergencies and changes in residents' conditions promptly and accurately; proficiency in using electronic medical records (EMR) systems for documentation and communication.

Working Conditions:
  • Environment: Work is performed in a Green House home setting, where the focus is on resident-centered care and a self-managed team model. The facility is designed to promote a comfortable, home-like atmosphere, supporting both staff and residents in a collaborative, nurturing environment. Exposure to bodily fluids, infectious diseases, and other health hazards, with the consistent use of personal protective equipment (PPE) required. Compliance with infection control protocols and health and safety standards is essential.
  • Physical Demands: Regular physical activity is required, including moving between residents' rooms, performing hands-on care, and assisting with physical tasks such as lifting, transferring, and repositioning residents. Ability to lift, move, or support residents and equipment, with the capacity to lift and/or move up to 50 pounds with appropriate assistance. Must be able to stand, walk, bend, squat, and engage in other physical tasks for extended periods.
  • Emotional Demands: The role requires emotional resilience and the ability to manage challenging or stressful situations, especially when dealing with resident needs, family concerns, and medical emergencies. Must maintain a compassionate and empathetic approach, providing care and support to residents and their families during emotionally sensitive moments. Ability to manage the emotional demands of working in a healthcare environment with a focus on person-centered care.
  • Hours and Shift: Nurses may work various shifts, including evenings, weekends, and holidays, to ensure continuous care and support for Elders. Flexibility in scheduling is essential to meet the needs of the Green House home.

Team Environment: The position involves working closely with a multidisciplinary team and the home's self-managed work team. Regularly partnering with Nurses, Care Partners, Guides, physicians, therapists, and other healthcare professionals, to provide comprehensive care. The role requires collaboration, communication, and shared decision-making with the team, promoting a culture of teamwork and support. The work environment is self-managed, meaning the team collectively handles day-to-day operations, resident needs, and staffing schedules. A supportive atmosphere is encouraged, where every team member contributes to the care of residents and the overall success of the home.
