We are currently seeking an Admissions Registered Nurse (RN) to join our Hospice team. This role offers the opportunity to build meaningful relationships with patients, work independently, and make a tremendous impact in the communityโ€”while receiving competitive pay and excellent benefits.

Job Summary

The Hospice Admission Registered Nurse (RN) serves as the first clinical point of contact for new hospice patients and families. This role conducts admission consultations and comprehensive assessments to determine hospice eligibility and develop individualized plans of care. The Admission RN delivers compassionate hospice nursing care in patients' homes and community settings while collaborating closely with the interdisciplinary team.

Key Responsibilities

  • Conduct comprehensive hospice admission assessments for patients and families
  • Perform physical assessments and obtain detailed medical histories
  • Determine hospice eligibility using assessment data and clinical judgment
  • Develop individualized plans of care based on nursing diagnoses and patient goals
  • Implement palliative and preventive nursing interventions
  • Administer medications and treatments per physician orders
  • Educate and counsel patients and families on care needs and comfort measures
  • Assist with activities of daily living (ADLs) while promoting comfort and independence
  • Communicate patient care needs with the hospice interdisciplinary team
  • Maintain accurate and timely clinical documentation

Qualifications

  • Current Registered Nurse (RN) license in California
  • Minimum of two (2) years nursing experience; hospice, home health, or public health preferred
  • Current CPR certification
  • Strong assessment, communication, and timeโ€‘management skills
  • Ability to work independently with minimal supervision
