Job description

The Triage Registered Nurse is responsible for reviewing all pertinent documentation (health histories, consultation notes, and diagnostic and laboratory tests) included with the referral to the Heart Investigation Unit. Based upon a review of this information they will appropriately triage the patient to the Cardiac Care Network (CCN) waitlist. The Registered Nurse will prepare to facilitate the booking of the procedure and follow-up care as well as management of the patient. Specific responsibilities include maintenance of a current, complete and accurate database for all patients awaiting HIU services. ย Assist referring physicians in triaging and scheduling of patients for HIU services (within and between institutions) to facilitate timely access to these services. Ensuring that patient/family focused support and educational information will be provided to patients/families. Liaise with referring centers to ensure that patient/family focused support and educational information will be provided. In participating as a member of the Cardiac and Vascular Patient Care Team, the Registered Nurse will collaborate and communicate with the HIU staff to ensure Best Practice and management of interventional cardiology patients. Communicating, collaborating and liaising with the HIU Operations Committee, Regional Cardiac Care Coordinators, the Cardiac Care Network (CCN) Steering Committee, CCN Provincial Office and the CCN Executive Director; working with the data entry clerk in ongoing data entry and management.ย 

1. Current registration with the College of Nurses as a Registered Nurseย 
2. Minimum 3 years' experience in cardiac care and Heart Investigation nursing within the last 5 years
3. Demonstrated critical thinking and triage skillsย 
4. Demonstrated administrative and computer skills with experience in word processing, spreadsheet and database applications
5. Demonstrated critical thinking skills excellence in interpersonal and delegation skills, communication skills (verbal and written) as well as prioritization, problem solving and organizational skills
6. Demonstrated ability to prioritize and handle multiple tasks and demands
7. Demonstrated ability to implement process improvements and systems that increase efficiency and effectiveness
8. Evidence of commitment to professional practice and lifelong learning
9. Demonstrated understanding of and ability to facilitate Heart Investigation/Interventional Cardiology service compliance with Cardiac Care Network and Ministry requirements
