Job description

RESPONSIBILITIES:

Complete Comprehensive Intake Assessment with each client, including baseline drug screen, brief interventions, development of individual treatment plan according to ASAM placement level, and participation in development of integrated treatment plan and the identification of appropriate level of care for treatment (ITT meetings).

Refer clients to appropriate services, i.e. medical, substance abuse, Detox, emergency food and shelter, HIV/AIDS prevention and treatment, entitlement programs, and all other company and community programs, as appropriate and in accordance with the integrated treatment plan.

Provide individual and group counseling/therapy, as assigned.

Participate in treatment plan updates, as assigned, according to ASAM placement level and client’s needs.

Facilitate substance use treatment groups and drug screens, as assigned.

Maintain current documentation in electronic data systems for each client and collateral contacts, and lock notes within 24 hours of encounter.

Participate in supervision and staff meetings as scheduled and training activities to maintain certification/licensure.

Ensure that follow-up contacts for No Shows and post-completions are completed and documented within required timeframe.

Assist in preparation of required monthly data and program reports for grants and contracts.

Participate in regularly scheduled case conferences, staffing, site visits, and multidisciplinary team meetings, and present as assigned.

All other duties as assigned.

Hours of Work: Forty hours/week ; evening hours as assigned.

Qualifications: Must possess , at a minimum, current licensure in The District of Columbia for LGSW or CAC. LICSW, LPC or LFMT preferred . Must have a minimum of two years of Behavioral Health experience and ability to assess for co-occurring disorders. HIV/AIDS experience desired. Proficiency with data entry in electronic data systems.
