  • Review case notes, service plans, assessments, and participant records for completeness, quality, and alignment with internal policies and funder requirements.
  • Ensure staff adherence to documentation timelines, data standards, and confidentiality requirements.
  • Collaborate with leadership to improve workflows, reporting processes, and system utilization.
Program Quality & Compliance
  • Monitor program implementation to ensure fidelity to grant requirements, internal policies, and service models.
  • Support program reporting, audits, and monitoring efforts by ensuring documentation readiness and data accuracy.
  • Identify trends, gaps, or risks in service delivery and recommend improvements to enhance participant outcomes.
Cross-Functional Collaboration
  • Serve as a key liaison between frontline staff and senior leadership, elevating operational insights and recommendations.
  • Contribute to program planning, evaluation, and continuous improvement efforts.
Community & Organizational Contribution
  • Participate in team meetings, training, and organizational planning sessions.
  • Support a positive, accountable, and mission-aligned team culture rooted in equity, respect, and transparency.
  • Represent Changing Perceptions professionally in internal and external settings as needed.
WHAT WE’RE LOOKING FOR:
Changing Perceptions may be the right fit for you if:
  • You have a Bachelor's degree in Human Services or Social Work.
