Job description

BASIC FUNCTION: Serves as a resource, an advocate and a partner for the individual, his or her family, others who are significant in his or her life, public agencies, and service providers. The Home Based Service Coordinator develops strategies that are responsive to individual needs, preferences, and satisfaction as well as promoting independence, productivity and community inclusion.
RESPONSIBILITIES:
1. Maintains an active caseload of at least 40 - 50 consumers.
2. Ensures and coordinates the provision of a holistic array of comprehensive services that are community-based, culturally relevant, and highly individualized.
3. Assures that the person receives necessary assessments in order to determine program eligibility.
4. Assists the individual and his/her representatives in the development of a service and support plan uniquely tailored to that person's individual needs, wants and preferences.
5. Ensures that the Interdisciplinary Team (IDT) is convened and that the person and his/her representatives are primary members of the team. The IDT must meet annually or as frequently as necessary to main the viability of the plan.
6. Determines whether services and supports exist, in the individual's own community, that can effectively meet his/her needs, preferences and satisfaction as identified in the ISP. In the event that such services and supports do not exist or are unable to meet identified needs, the individual service coordinator will actively broker these specific services and supports desired by the individual as stated in the ISP.
7. Ensures that all services and supports are received in accordance with the plan. In the event that the individual does not consistently receive the quality of service and supports desired, the Home Based Service Coordinator will assist him/her in initiating actions to overcome the identified barriers.
8. Reviews service data and information to ensure that it is accurately recorded and evaluated. Specifies actions taken in response to such reviews and steps taken to facilitate the person's progress and satisfaction.
9. Collects information about their satisfaction with existing services and supports and determines what additional services and supports are needed and wanted.
10. Advocates for the person in securing needed and preferred services and supports.
11. Assists the individual in exercising rights and choices available under state and federal laws.
12. Maintains all required documentation in accordance with specified timeframes, SCPF, Inc. policies, DHS-DD standards and in conformance with the confidentiality Act.
13. Visits each individual on assigned case load at the frequency defined by the program parameters.
14. Demonstrates the ability to relate to persons with developmental disabilities and their families in a manner that strengthens, promotes, and empowers the individual in determining the most effective use of resources to meet his/her life span needs and preferences, regardless of the severity or level of disability.
15. Submits statistical reports for input into the states ROCs reporting system monthly. Utilizes the ROC's system and SCPF, Inc. consumer database to maintain computer files and records.
16. Executes all required billing for the Home-Based Support and Family Assistance Programs by the last business day of each
month.
17. Reconciles all billing reports for the Home-Based Support and Family Assistance Programs monthly.
18. Maintains a flexible schedule which may include evenings and/or Saturday hours.
19. Demonstrates knowledge of systems resources as well as generic resources in order to ensure the provision of a balanced delivery of services.
20. Performs other relevant program duties as assigned.
Requirements
1. Bachelor's Degree in human services field, including, but not limited to Sociology, Special Education, Rehabilitation Counseling, Psychology, or Social Work, and experience as a QIDP.
2. Aptitude for developing relationships with adults with developmental disabilities.
3. Good writing skills.
4. Familiarity with available and potential resources within the community served by the agency.
5. Demonstrates considerable initiative, self-directedness, resourcefulness and a firm commitment to maintaining people with disabilities in the community.
6. Maintain a flexible work schedule.
7. Maintain a valid driver's license; has an acceptable driving record as determined by agency auto insurance carrier (insurable under agency auto policy); has a reliable, insured car available to use during work hours.
