As a Social Worker with WesleyLife At Home, you will:

  • Explain WesleyLife Hospice and Palliative services to patients and families.
  •  Visit patient homes (wherever they call home) to assess the psychosocial status, safety concerns, and bereavement risk factors of patients and families/caregivers.
  • Communicate findings to patient care teams to update care plans.
  • Carry out social evaluations and plan interventions.
  • Counsel patients and families/caregivers regarding stress and other identified coping difficulties.
  • Provide information and referral services for patients and families/caregivers regarding financial, practical, and environmental needs.
  • Serve as liaison with patients and families/caregivers and care providers or community agencies.
  • Participate in caregiver, placement, and discharge planning when needed, including discharge to a higher level of care.
  • LISW preferred, LMSW highly encourage to apply.
