Hiring Range

$18.90 - $24.60/Hourly Actual pay is determined based on job-related factors such as relevant experience, education, credentials, skills, internal equity, and business needs.


Overview
Works under the supervision of the Charge RN/Nursing Supervisor/Nurse Manager/Director in personal care of the patients and their environment, including those needing special precautions. This position must be able to demonstrate the knowledge and skills necessary to support/provide care appropriate to the age of the patient/resident served on unit assigned. The individual must have knowledge of the principles of growth and development over the life span and possess the ability to assess data reflective of the patient's/residents status and interpret the appropriate information needed to identify each patient's/resident's requirements relative to age-specific needs and to provide the care needed as described in the departmental policy and procedure guide.
What you will do

  • Assesses and treats children, adolescents, and adults seeking psychiatric and/or chemical dependency treatment. Communicates patient, resident, family needs to physicians accurately and completely. Familiarizes self with the plan of care and provides relevant insight into overall success or areas of improvement.
  • Collaborate with referral sources, patients, families, and team members regarding care coordination. Provides follow-up actions as needed.
  • Serves as the initial contact for patients, families, referral sources and/or allied agencies seeking intervention or support. Provides complete and accurate Intake Screening Reports of the patient and/or caller complaint and condition, including initial observation made upon arrival, reflective of the patient's status and age, as described in the policy and procedure guide. Completes vital sign checks, mini-mental status exams, belonging checks.
  • Leads patients in therapeutic and recreational activities.
  • Monitors patients' physical and emotional wellbeing and reports unusual behavior or physical ailments to the medical staff.
  • Is able to observe patients for safety as directed by RN/ LPN to include rounding, direct observation, sitters
  • Assists patient with activities of daily living to include hygiene, laundry, dressing, personal care as needed.


Qualifications
Education

  • High School Diploma or GED, (Preferred)


Experience

  • 1 year Behavioral health experience (Preferred)


Licenses and Certifications

  • CPR/BLS Certification - American Heart Association/American Red Cross/American Safety and Health Institute (AHA/ARC) within 30 Days(Required) and
  • First Aid - American Heart Association/American Red Cross/American Safety and Health Institute (AHA/ARC) within 30 Days(Required)
