Position Summary - Field-Based Case Manager (RN / Behavioral Health Clinician) - LTSS

Join Aetna's Long-Term Services and Supports (LTSS) team in a dynamic, field-based Case Manager role where you can make a meaningful difference in the lives of members throughout Virginia. This position is ideal for a compassionate Registered Nurse or Behavioral Health Clinician who is passionate about delivering personalized, community-centered care and thrives in a role that blends autonomy, travel, and collaboration.

As a Case Manager, you will take the lead in assessing, planning, and coordinating comprehensive care for members with diverse medical and behavioral health needs. Through in-person visits (up to 75% travel), you'll build strong relationships, gain deeper insights into members' environments, and create tailored care plans that promote both immediate health improvements and long-term wellness.

You will play a critical role in integrating services, addressing barriers to care, and connecting members with appropriate resources across healthcare and community systems. By working closely with interdisciplinary teams, providers, and support networks, you'll ensure care is proactive, coordinated, and aligned with each member's unique goals.

  • Must reside in Norfolk VA or Surrounding Areas.
  • Must be willing and able to travel up to 75% of the time to meet members in their home

Preferred Qualifications

  • Case Management in an integrated model preferred
  • Bilingual in English/Spanish preferred
  • Ability to multitask, prioritize and effectively adapt to a fast-paced changing environment
  • Position requires proficiency with computer skills which includes navigating multiple systems and keyboarding.
  • Effective communication skills, both verbal and written.

  • Associate degree in Nursing required
  • Bachelor's degree preferred

The typical pay range for this role is:

$60,522.00 - $129,615.00

This pay range represents the base hourly rate or base annual full-time salary for all positions in the job grade within which this position falls. The actual base salary offer will depend on a variety of factors including experience, education, geography and other relevant factors. This position is eligible for a CVS Health bonus, commission or short-term incentive program in addition to the base pay range listed above.
