Job description

Position Overview

Hampton Roads Community Health Center is looking for a Nurse Practitioner to join our clinical team. We are strategically located in the Hampton Roads area of Virginia. Hampton Roads is a metropolitan area in southeastern Virginia that includes the cities of Chesapeake, Hampton, Newport News, Poquoson, Suffolk, Virginia Beach, and Williamsburg. It also includes the counties of Gloucester, Isle of Wight, James City, Mathews, and York, and Currituck County, North Carolina. The Hampton Roads region is known for its connection to water, and includes the Chesapeake Bay, the Atlantic Ocean, and many rivers. It's also home to one of the world's largest natural harbors and is the northernmost major East Coast port of the United States that's ice-free year-round. 

Under general supervision, this exempt position is responsible for providing diagnosis and treatment of acute and chronic health problems along with preventive care focused on health risk factor reduction.  Reports to the Chief Medical Officer (CMO).  

Key Responsibilities

  • Provides primary care services of an acute, chronic, and preventive nature to patients of all age groups as guided by organizational protocols and license to practice; diagnosis and treatment of common acute and chronic illness, including minor trauma, episodic problems, common chronic disorders, as well as counseling and education with regard to such problems; and performance of minor surgical procedures and initiation of life-saving procedures in emergencies. 
  • Plans diagnostic studies for each patient as necessary; evaluates all diagnostic results; consults with supervising physician as appropriate; writes prescriptions for required medication and/or treatment to meet the management plan, as allowed by state regulation and/or clinical protocols; refers complex and high priority cases to the physician on duty with regard to complicated diagnostic problems, serious illness, complicated therapeutic problems, reevaluation of chronic conditions, etc.
  • Completes Electronic Medical Records according to practice standards and closes charts within the appropriate time frame; serves as a preceptor for medical and nursing professionals and other students as reasonably assigned; writes referrals and works with other staff to effectively communicate information to patients and their families; and utilizes medication assistance services as appropriate. 
  • Performs administrative support work such as word processing, creating spreadsheets, data entry and retrieval; orders, monitors and distributes program supplies/resources; and other information to ensure accuracy and conformance to established procedures and policies; and counsels patients regarding patient responsibilities and rights; adheres to patient confidentiality guidelines. 
  • Performs other duties as assigned.

Required Qualifications

  • Licensed to practice as Nurse Practitioner in the Commonwealth of Virginia, with prescriptive authority and three (3) to five (5) years of related experience is preferred.  Basic Life Support Certification. Family Nurse Practitioner Certification is desirable. Newly Licensed Nurse Practitioners are strongly encouraged to apply.
