Job description

GT Independence is a family-founded, leading national Financial Management Services (FMS) provider that supports individuals participating in self-direction programs, helping them manage their long-term care and support services at home and in community-based settings. With a growing national presence, the organization continues to expand its impact through innovation, service excellence, and a strong commitment to the people and communities it serves.

The Care Integration Senior Aide (CISA) conducts in-home visits to work directly with clients and Direct Support Professionals (DSPs). The CISA observes, documents, and reports changes in clients’ health and social conditions, collaborates with the care team to help prevent avoidable negative outcomes, and supports quality service delivery throughout the Community Supported Living (CSL) program.

Responsibilities

  • Conduct in-home visits to observe and document clients’ health and social needs.
  • Support and coach Direct Support Professionals (DSPs) in service delivery and client engagement.
  • Communicate observations, concerns, and updates to the interdisciplinary care team.
  • Complete follow-up activities, maintain member records, and support care plan implementation.
  • Train and guide DSPs on approved supports and evolving client needs.
  • Build positive relationships with clients, families, caregivers, and community partners.
  • Serve as a resource to DSPs and promote person-centered, high-quality care.
  • Adhere to agency policies, professional standards, and documentation requirements.
  • Provide direct support to CSL members as needed within the scope of the role.

This position provides guidance and support to DSPs but does not have supervisory responsibilities, including scheduling, performance management, or disciplinary functions.

Qualifications

  • High school diploma or equivalent.
  • One year of caregiver experience with the agency preferred, or two years of caregiver experience in a similar setting.
  • Reliable transportation, a valid driver’s license, and proof of current automobile insurance.
  • Ability to speak, read, and write functional English sufficient to perform essential job duties and complete required documentation.

As an organization focused on helping both the people we serve and our employees succeed, we foster a culture where individuals can grow, thrive, and make a meaningful difference every day. Join us in our mission: "To help people live a life of their choosing regardless of age or ability."

GT Independence is an equal opportunity/affirmative action employer. All qualified applicants will be considered without regard to age, race,
color, sex, religion, nation origin, marital status, ancestry, hairstyle, citizenship, veteran status, sexual orientation or preference, or physical or
mental disability.
