This position involves an onsite work setting, primarily focusing on home health care services. You will be visiting patients in their residences, delivering personalized treatments directly in their environments. • Conduct comprehensive evaluations of patients by reviewing pertinent medical histories, performing necessary assessments, and meticulously documenting results and recommendations.
• Develop and implement individualized treatment plans, effectively communicating evaluation findings and objectives to both patients and their families to foster teamwork in goal-setting.
• Demonstrate advanced skills in applying treatment protocols that align with the patient’s functional status and evolving health care standards.
• Collaborate with other healthcare professionals to enhance patient care, recognizing and documenting any changes in the patient’s condition that may require adjustments to the treatment plans.
• Manage discharge planning effectively and ensure timely follow-up care, maintaining rigorous documentation of patient progress and care activities.
• Exhibit strong time management to meet productivity metrics while ensuring patient schedules are maintained accurately.
• Contribute to department goals by participating in quality enhancement initiatives and adhering to all regulatory and accreditation guidelines.
• Pursue continuing education opportunities for both personal and team development, and mentor students as a clinical instructor.
• Promote a positive patient experience by offering flexible solutions and demonstrating adaptability to individual patient needs.
• Engage actively in team-driven initiatives aimed at optimizing patient access to care, utilizing CSI measurement data to inform efforts.

This role primarily serves a diverse patient population, including elderly individuals recovering from surgeries, stroke patients, and those with chronic conditions requiring targeted rehabilitation. Your ability to assess and understand the unique dynamics within each patient’s home environment will be beneficial in designing effective care strategies that promote independence and restore functional movement.

• A Bachelor’s degree in Physical Therapy from an accredited institution.
• A current state license to practice as a Physical Therapist as required by Virginia state regulations.
• Certification in Basic Life Support (BLS) for Healthcare Providers by the American Heart Association.

While prior work experience is not mandatory, we value any relevant clinical rotation or internship experience that equips you with practical skills and familiarity with patient-driven therapy.
