Work Environment

Key Responsibilities
  • Conduct comprehensive evaluations of patients using established protocols, which include reviewing medical history, performing assessments, and documenting findings accurately.
  • Design and explain tailored treatment plans to patients and their families, incorporating their input in goal-setting to promote adherence to the therapeutic process.
  • Implement and supervise treatment protocols based on clinical assessments, modifying them according to the evolving needs of patients.
  • Engage with other healthcare professionals to ensure integrated patient care, documenting changes in patient condition that may alter the course of treatment.
  • Oversee discharge planning and follow-up procedures, ensuring clear, legible documentation that tracks patient progress, interim evaluations, and final assessments.

  • Participate in initiatives aimed at removing barriers to patient access, actively engaging in the Continuous Improvement System (CSI) to enhance service delivery.
Patient Population / Client Focus
