Job description

Crosspoint Human Services is hiring caring, dedicated professionals to provide quality, person-centered support to individuals with diverse needs, including developmental disabilities, mental illness, and other health conditions. In this vital role, you will empower individuals to live as independently as possible while ensuring their safety, comfort, and well-being.

Responsibilities and Duties:

Duties include, but are not limited to:

  • Complete comprehensive assessments and collaborate with individuals served to develop a care plan that is tailored to their unique needs and goals.
  • Provide assistance and ongoing support for obtaining and maintaining a safe, stable living environment.
  • Provide or arrange for employment, vocational support, and skills training.
  • Provide or assist in arranging for needed support services.
  • Work closely with members of the interdisciplinary team to coordinate care.

Job Requirements

  • Requires a Bachelor's degree in counseling or related field.
  • Knowledge of serious mental illness, addictions and developmental disabilities.
  • Excellent assessment, counseling and training skills.
  • Valid driver's license and proof of automobile insurance.
