Key Responsibilities
  • Conduct comprehensive evaluations, including reviewing patients’ medical histories, performing assessments to identify physical impairments, and establishing personalized treatment plans that align with patients’ goals.
  • Effectively communicate evaluation findings and treatment plans to patients and their families, ensuring their involvement in the goal-setting process.
  • Implement treatment protocols with precision, adapting therapeutic interventions based on each patient's evaluation results and ongoing feedback.
  • Collaborate with other healthcare professionals to coordinate care, recognizing when adjustments to the treatment plan are needed based on changes in patient conditions.
  • Manage discharge planning and follow-up care, ensuring all documentation is completed timely and accurately—this includes detailed notes and progress summaries that reflect the patient's recovery journey.
  • Maintain productivity and manage time effectively; attend meetings and engage in community outreach, enhancing the visibility of our clinic.
  • Engage in departmental goal-setting, ensuring compliance with governmental and accreditation standards while contributing to the development of the department.
  • Pursue continuing education opportunities to enhance personal professional growth and serve as a clinical instructor for students.
  • Promote exceptional customer experiences, demonstrating flexibility and adaptability in various patient interactions and managing access initiatives.
Patient Population / Client Focus

You will work closely with a diverse set of patients, including those recovering from surgeries, injuries, chronic pain, and various musculoskeletal issues. Your role will be pivotal in aiding their recovery, improving their mobility, and enhancing their overall quality of life.

Qualifications
  • Must possess a Doctorate Degree in Physical Therapy (DPT).
  • Valid Physical Therapy license in the State of Connecticut.
  • Current certification in Basic Life Support (BLS) for Healthcare Providers from the American Heart Association.
  • Strong interpersonal and communication skills to effectively engage with patients and team members.
Preferred Experience
