Position: CRED NYC Case Manager, SkillSpring
Location: Multiple Sites in Manhattan & the Bronx
Salary Range: $ 65,000 to $70,000 per annum
Reports to: SkillSpring’s CRED NYC Program Director

SkillSpring, a workforce development program of The New Jewish Home, has supported New York City youth and young adults from under-resourced schools and communities in pursuing careers in health care since 2006. By training the next generation of high-quality, culturally competent health care professionals, SkillSpring generates excitement and enthusiasm for careers serving older adults.
Position Overview:

SkillSpring is seeking a Case Manager for our Community Resources for Employment and Development Program (“CRED NYC”) Program. The Case Manager will work directly with trainees participating in our CRED NYC program that includes work readiness, Certified Nursing Assistant (CNA) classroom-based course instruction, and onsite clinical and professional training . The Case Manager will focus on trainees’ mental health, well-being, and self-management, providing services that support trainees’ needs through small groups and one-on-one sessions as needed. This full-time role requires flexibility in working mornings, evenings, and Saturdays at multiple sites, with hours, days, and locations varying based on seasonal changes.
Key Responsibilities:

• Design Trauma-Informed Workshops
• Create Trauma-Informed information sheets for case managers
• Plan, Conduct, and Deliver Trauma-Informed Workshops
• Distribute and collect pre-and post-Trauma-Informed surveys
• Conduct community outreach to increase workshop attendance
• Provide assistance to workshop attendees in need of social service support including mental health referrals 
• Follow up on all referrals 
• Document all activities, including workshop events and referrals
• Provide clinical support for staff to better assist trainees
• Cultivate and maintain relationships with key strategic partners in education, career development, intergenerational programming, and youth development to ensure trainee enrollment and retention
• Ensure accurate collection, analysis, and management of data for internal and external reporting, improving mechanisms to ensure accountability and transparency
• Develop and enhance tools to support SkillSpring in generating employment and economic opportunities, including communications collateral, reporting templates, policies, and protocols
• Adherence to all CRED NYC contract regulations
• Perform other related duties as required
