You will work onsite at our rehabilitation facility in Greeley, where a supportive and collaborative atmosphere fosters professional development. Our facility is equipped with state-of-the-art rehabilitation tools and technologies, ensuring you have the necessary resources to provide the highest level of care. The clinic operates from Monday to Friday, from 9 AM to 5 PM, allowing for a well-rounded work-life balance.

• Conduct thorough evaluations of patients' physical capacities by utilizing standardized assessments to determine their unique needs and goals.
• Create and implement individualized treatment plans that incorporate therapeutic exercises, manual therapy, and cutting-edge modalities tailored specifically for geriatric patients.
• Continuously monitor and document patient progress, making timely adjustments to treatment plans to achieve optimal outcomes in collaboration with our healthcare team.
• Engage in interdisciplinary collaborations with physicians, occupational and speech therapists to deliver comprehensive patient care.
• Educate patients and their families on therapeutic techniques, effective home exercise programs, and strategies for preventing injuries, thereby fostering empowerment and independence.
• Design and implement fall prevention programs tailored to the assisted living community to mitigate risks and ensure patient safety.
• Stay updated with the latest advancements in physical therapy practices through ongoing professional development and continuing education opportunities.
• Uphold ethical and professional standards, ensuring confidentiality and the delivery of high-quality care in all interactions.

Our primary focus is on the geriatric community, including seniors living independently, those in assisted living facilities, and patients recovering from acute conditions. You will provide therapy that respects the unique needs of older adults, working to empower them to regain and maintain their independence. • An accredited Bachelor's degree in Physical Therapy, with a strong preference for candidates holding a Doctorate or Master's degree.
• Current and valid state licensure as a Physical Therapist.

• Excellent interpersonal and communication skills to effectively interact with patients, families, and colleagues.
