Cowlitz Family Health Center is a Federally Qualified Health Center (FQHC) currently seeking a Nurse Practitioner to join our team. In this role, you will assume the responsibility of delivering primary care medical services in a manner that is professional, optimistic, consumer-friendly, and rooted in evidence-based practices. As a pivotal member of our collaborative care team, you'll work alongside medical assistants, receptionists, referral and records experts, care coordination professionals, behavioral health specialists, and outreach and enrollment staff.

Your role will encompass a range of duties, including diagnosing medical conditions, prescribing medications, monitoring potential medication side effects, and providing primary care and preventive health treatments. Requirements:
  • Active Licensure to practice in the State of Washington, without restriction, probation, or limiting condition or threat thereof
  • Board certified or Board eligible in Family Practice, Internal Medicine, or Med/Peds
  • Possess a valid, unrestricted license or registration to prescribe drugs, medications, pharmaceuticals, and controlled substances
  • Never been disciplined by any professional or peer review organization, governmental agency, licensing body or hospital medical staff for any action or omission based on or related to quality of care.
  • Knowledge of evidence-based primary clinical care standards.
  • Demonstrated ability to collaborate effectively in a team setting.
  • Ability to maintain effective and professional relationships with patients and other members of the care team.
  • Strong communication skills.
  • Ability to effectively engage patients in improving and maintaining their health
  • Experience working with underserved, transient populations.
  • Proficient computer skills and experience with electronic health records

Desired Knowledge and Skills:

  • Two or more years related experience providing primary care in an outpatient setting.
  • Bilingual English/Spanish.
