Directly participate in household maintenance activities when required.Provide coverage at all residential sites as needed.Provide training, supervision and assistance to residents in daily activities including food purchases, recreational activities, and meal planning and personal care activities. Assist residents in developing meaningful service plan goals and provide supports to assist residents in achieving their goalsConduct fire safety training activities and ensures fire drills are conducted in accordance with regulatory guidelines.Participate in all Medicaid billable activities under leadership of the Supervisor, and/or ensure that all documentation required for the particular program is current and in compliance with regulatory guidelines. Document interactions with residents in accordance with agency policies and standards.Meet weekly, or as scheduled, with assigned primary residents for counseling and to establish and review service plan goals.Maintain collateral contacts with outside service providers and participate in case conferences as scheduled.Assist residents in procuring and maintaining financial entitlements.Monitor medications as required.Make visits to residents in apartments as scheduled and/or provide 24-hour supervision to residents in congregate facilities.Provide transportation as needed using personal vehicle when agency vehicle is not available.Promptly report all emergencies, incidents, maintenance and safety issues to senior Counselor or Supervisor.Participate in team meetings as scheduled.Complete admission documentation post-intake for new clientsFacilitate sharing of data with the individuals to whom it applies to facilitate partnered decision-making and to keep clients informed of progressComplete QI Training during onboarding and participate in QI refresher training as needed.Ability to cultivate a culture of inclusion for all employees that respects their individual strengths, views and experiences.
