Job Overview:

We're seeking a compassionate and driven Social Services Designee to support residents, families, and care teams with dignity, empathy, and purpose. In this role, you won't just complete tasks-you'll help shape quality of life, protect resident rights, and ensure every individual is seen, heard, and supported. This position is located at Deaconess Midtown or Gateway campus.
Position Requirements:
BSW Case Manager
  • Graduate of a program accredited by the Council on Social Work Education with a Bachelor of Social Work (BSW).
  • Recommended to continue education with obtaining Masters in Social Work (MSW) - Deaconess will assist in continued education through tuition reimbursement program.
  • Experience in health care, crisis intervention, counseling and/or social agency preferred. This can include internships or other employment.

MSW Case Manager
  • Graduate of a program accredited by the Council on Social Work Education with a Masters of Social Work (MSW).
  • Experience in health care, crisis intervention, counseling and/or social agency preferred. This can include internships or other employment.

Shifts: Monday- Friday
