Position Overview

Back on Track Counseling & Consulting is seeking a dedicated and compassionate Licensed Clinical Social Worker (LCSW) to join our team in a full-time remote role. This position is integral to our commitment to providing exceptional mental health care, specifically within a detox center environment where patients confront both physical and psychological challenges. You will play a pivotal role in assessing, supporting, and guiding patients on their journey to recovery.

Key Responsibilities
  • Conduct comprehensive psychosocial assessments of incoming patients, identifying individual strengths, needs, and barriers to recovery.
  • Provide tailored counseling sessions focusing on mental health challenges and accompanying substance use issues, utilizing evidence-based therapeutic modalities.
  • Collaborate with a multidisciplinary team to develop, implement, and adjust personalized care plans that target behavior modification and adaptive coping strategies.
  • Encourage goal setting for patients, aiding them in managing chronic conditions and enhancing their mental health through synchronized strategies.
  • Engage patients with regular outreach and support, adapting your approach based on their preferences and progress to catalyze behavior change.
  • Document all patient interactions and care plans accurately in our electronic health record (EHR) system, ensuring compliance with regulations and continuity of care.
Patient Population / Client Focus

You will work with a diverse population of individuals recovering from substance use disorders, many of whom face co-occurring mental health issues. Your insight and sensitivity to varied cultural backgrounds will enhance the therapeutic relationship and improve overall treatment success rates. Qualifications
  • Master's degree in Social Work from an accredited institution.
