General Purpose:

Under the supervision of the Senior Program Supervisor, the Community Outreach Worker provides monthly nutrition health education sessions, distributes Food Pantry Bags and Food Vouchers, arranges medical transportation rides, ensures program and ride destination eligibility, tracks transportation status, as well as field consumer, provider and vendor concerns, as they arise.


Key Competencies:

  • Coordinate timely provision of services for clients so that they can access HIV-related health and support services.
  • Coordinate and maintain multi-county vendor/provider transportation networks to ensure that they are following agency standards and requirements.
  • Maintain accuracy of program documentation to comply with program/funder standards.
  • Conduct program intakes, screenings and facilitate client enrollment into the program.
  • Identify needs and complete referrals to community partners with whom the program has established cooperative linkages, such as HIV service organizations, case management providers, and other relevant community resources to ensure clientsโ€™ needs are being met.
  • Outreach to other agencies and medical facilities in the multi county area making them aware of program services.

Description of Duties:

  • Conduct outreach to promote program services to PLWH and to community providers.
  • Screen referred consumers for eligibility and assess needs.
  • Provision of food pantry/food vouchers to clients.
  • Process ride requests and arrange transportation according to the programโ€™s policies and procedures.
  • Track the status of transportation services to ensure proper service delivery and appropriate follow-up, as needed.
  • Develop a system to verify that clients attend medical or Ryan White fundable services or appointments as intended.
  • Assist in the adherence to program guidelines related to the compilation of pantry bags, including the modification of contents according to clientsโ€™ nutritional needs and cultural preferences.
  • Collect and monitor food voucher receipts from clients for compliance with program requirements.
  • Assist with troubleshooting/following-up on complaints and incident reports; vendor billing reconciliations; monthly report preparation and identify new vendors when needed
  • Attend and participate in monthly department, team, All Staff and other required meetings.
  • Be familiar with Cornerstone policies and procedures and the Employee Handbook.
  • Maintain confidentiality of all aspects of Cornerstone including, but not limited to, patient confidentiality, financials, and employee relations.
  • Perform other related duties as assigned.
Requirements:
  • H.S. diploma. 1 year experience working in the field of HIV/AIDS, behavioral health, substance use or other chronic illnesses; or
  • Associates Degree or Bachelorโ€™s Degree in Human Services related field, with 1 year experience working in the field of HIV/AIDS, behavioral health, substance use or other chronic illnesses.
