Position Summary:

The Private Duty RN is responsible for delivering individualized, skilled nursing care to patients in their homes. This includes clinical tasks, daily living assistance, and collaboration with physicians and care teams to ensure optimal outcomes for medically fragile patients.

Patient Care & Clinical Duties:

Implement the physician’s plan of care and nursing care plan.

Provide safe, high-quality nursing care and monitor for changes in patient condition.

Accurately document clinical/progress notes and summaries.

Support and teach patients appropriate self-care techniques.

Assist with activities of daily living (ADLs) including bathing, grooming, and hygiene.

Maintain a clean, safe environment and use universal precautions.

Support or perform tracheostomy care, tube feedings, ventilator use, and oxygen therapy (with competency).

Medication Management:

Ensure medications are stored and administered according to physician orders and protocols.

Maintain accurate Medication Administration Records (MAR).

Observe for and report side effects or issues with medication administration.

Perform and document narcotic counts at shift start and end (for clients using controlled substances).

Follow procedures for reporting med errors and drug disposal.

Hygiene & Equipment Maintenance:

Provide or assist with hygiene care (oral care, shaving, bathing, incontinence care).

Monitor and document bowel movements, skin conditions, and rashes.

Maintain cleanliness of wheelchairs, prosthetics, and adaptive devices.

Ensure nails are trimmed and clean; observe for signs of skin breakdown.

Qualifications:

Current RN license in the state of practice (DC, MD, or VA)

1+ year of experience as an RN in a home care setting

Graduate of an accredited RN nursing program

Current CPR certification required

Valid driver’s license and reliable transportation

Strong interpersonal, communication, and documentation skills

Flexibility in scheduling and ability to travel locally
