POSITION SUMMARY โ€“ RN Case Manager

POSITION DUTIES โ€“ Coordinate patient care from admission through discharge, ensuring that treatments, services, and resources align with each patientโ€™s medical needs and recovery goals. Collaborate closely with physicians, nurses, and external providers to remove barriers to care, streamline transitions, and support safe, efficient discharge planning.

MINIMUM REQUIRED QUALIFICATIONS โ€“ย 

  • TX/compact RN license
  • BLS
  • 2+ years minimum of acute care hospital experience in a case management
  • Acute hospital experience in specialty during the past year with no more than a 90-day break

LENGTH OF ASSIGNMENT โ€“ 13 weeks

SHIFT / HOURS PER WEEK โ€“ 8A-4:30P

SYSTEMS โ€“ Meditech

START DATE โ€“ ASAP
