Avail Health – Contract PRN Mobile Nurse Practitioner – West Virginia, South Region

Avail Health partners with Medicare Advantage health plans to support Medicare‑eligible individuals, including those in long‑term care settings, through clinically driven programs focused on proactive care, accurate clinical documentation, and improved health outcomes. Our work is grounded in empowering clinicians, reducing administrative burden, and enabling consistent, high‑integrity care delivery that enhances both patient experience and care team effectiveness.

About the Role

This 1099 contract position supports Avail Health’s partnership with an Institutional Special Needs Plan (I‑SNP) Medicare Advantage health plan. As a mobile Nurse Practitioner, you will conduct in‑person Chronic Disease Assessments using data provided by the health plan and evaluate members in long‑term care facilities to support accurate clinical documentation of chronic conditions. You will also complete Health Risk Assessments with the member and their authorized representative (if applicable) to evaluate health risks and develop Individualized Care Plan recommendations. These assessments and care plan recommendations are used by the health plan to support coordination with the interdisciplinary care team responsible for the member’s ongoing care.

This highly collaborative, patient‑facing role requires strong clinical judgment, effective communication, and close partnership with Avail Health operations to support scheduling, documentation, and overall program execution. Comfort working independently in the field is essential, along with flexibility and a willingness to contribute ideas and feedback to support continuous improvement in a dynamic, growth‑oriented environment.

Contract Details

Contract Term: Engagement begins upon execution of the agreement and continues through June 30, 2026. Thereafter, the engagement may be extended in successive 3‑month increments aligned to calendar quarters, based on program needs and performance.

Schedule Commitment: This is a PRN (as‑needed) opportunity designed to offer flexibility based on your availability and local patient needs. Service volume may vary depending on patient census and regional demand, with anticipated utilization averaging approximately 2 days per week within your assigned geographic area. Contractors can designate their available weekdays (Monday–Friday, 8:00 a.m.–5:00 p.m.) for completing on‑site patient visits. Availability is submitted in advance to support monthly scheduling, with visits aligned to clinician availability and program visit targets.

Position Type: Mobile role supporting patients across the Southern region of West Virginia, including Beckley, Lewisburg, Bluefield, and Gary, with a preference for candidates based for central access across the region.

Compensation
  • Hourly Rate: $85/hour (1099 independent contractor)
  • Mileage Reimbursement: $0.725 per mile for travel exceeding 30 miles per day, up to $200 per day
  • Training Rate: $50/hour for required training and program‑related meetings, including compliance, technology, and clinical documentation
Meeting Expectations

To support strong clinical alignment and team collaboration, meetings may be scheduled Monday through Friday between 8:00 a.m. and 5:00 p.m. Time spent in these meetings is compensated at the standard Training Rate. When meetings fall within a clinician’s submitted availability, schedules will be adjusted to allow for participation. When they occur outside of availability, recordings will be provided for review during the workday, along with any applicable follow‑up items to support ongoing alignment.

Required Qualifications
  • Master’s or Doctoral degree in Nursing (MSN or DNP) from an accredited program
  • Current national certification as a Nurse Practitioner (FNP or Adult‑Gerontology NP)
  • Active, unrestricted Nurse Practitioner and Registered Nurse licensure in the state(s) of practice, maintained in good standing and in compliance with all applicable state‑specific practice regulations throughout the engagement
  • Minimum 3+ years of Nurse Practitioner experience working with older adults and/or chronic disease populations; experience in primary care, geriatrics, value‑based care, mobile care, or long‑term care settings preferred
Skills and Competencies
  • Strong clinical assessment, diagnostic, and patient management skills
  • Experience with care planning and interdisciplinary coordination preferred
  • Ability to work independently in a mobile setting while maintaining strong communication with care teams and operational partners
  • Excellent communication and interpersonal skills with patients, families, and interdisciplinary teams
